him or on his behalf due to or on account of Claim No. 07-806 ] 82 shall cease and forever <br />terminate effective April 27, 2012. <br />The parties further agree that Plaintiff=Appellee, David M. Vet, shall submit any and aIi <br />bills for medical, surgical, hospital, pharmacy, psychiatric/psychological, rehabilitation and <br />nursing expenses rendered to him or on his behalf due to or on account of Clairn No. 07-806182 <br />on or before April 27, 21012 within thirty (30) days of Apri127, 2012 to Defendant-Appellant, <br />City of Lakewood, or its agent for payment. Otherwise, Plaintiff-Appellee, David M. Vet, shall <br />be solely responsible for the payment of all such services. <br />The parties further agree that wherever City of Lakewood is used hereinafter in this <br />Settlement Agreement and Release, it shall mean and include Lakewood. <br />Plaintiff Appellee, David M. Vet, shall, except as specifically otherwise provided herein, <br />be solely responsible for the payment of any and all unpaid medical, surgical, hospital, pharmacy, <br />psychiatric/psychological, rehabilitation and nursing expenseslservices for services rendered to <br />him or on his behalf due to or on account of Claim No. 07-806182, as well as and including those <br />related to or on account of any other disease, condition or injury, allowed or disallowed, prior, <br />present or future, which have or which may or which could have been submitted to Defendant- <br />Appellant, City of Lakewood, or to any agent of said Defendant-Appellant, City of Lakewood, or <br />to the Hureau of Workers' Compensation or to the Industrial Commission of Ohio for payment <br />but which have not been paid and, further, that Plaintiff-Appellee, David M. Vet, shall hold <br />Defendant-Appellant, City of Lakewood, harmless for any and all such unpaid medical, surgical, <br />hospital, pharmacy, psychiatriclpsychological, rehabilitation and nursing expenses/services that <br />he has or may have incurred at any time as the result of or attributable to any and all numbered or <br />-2- <br />
