unnumbered claims including, but not limited to Claim No. 07-806182, as well as and including
<br />those related to or on account of any other disease(s), condition(s) or injury(ies) allowed or
<br />disallowed, which Plaintiff-Appellee, David M. Vet, has, might or could have filed with the
<br />Industrial Commission of Ohio, the Ohio Btueau of Workers' Compensation and/or with City of
<br />Lakewood or any agent of City of Lakewood The following claims are specifically Excluded
<br />from the terms and provisions of this Settlement Agreement and Release: Any claim which has
<br />been filed as against the City of Lakewood with a date of injury prior to March 1, 2006 and any
<br />claim with a date of diagnosis prior to IvZarch 1, 2006 as well as any claim which has, may or
<br />could have been filed as against any other employer. Defendant-Appellant, City of Lakewood,
<br />will pay the court costs in Case No. CVD-2011-0062.
<br />The said overall award of One hundred l='ifty Thousand Dollars ($150,000.00) without
<br />interest will be accepted by Plaintiff-Appellee, David M. V et, the sufficiency of which is hereby
<br />acknowledged, in full and complete satisfaction, settlement and discharge of the cause of action
<br />herein, and of any and alt other claims, demands, actions, charges, liabilities, rights or causes of
<br />action, of whatever nature, description or type, whether legal or equitable, which Plaintiff-
<br />Appellee, David M. Vet, has, claims to have or could have filed or alleged, whether known or
<br />unknown, including, but not limited to, any claims to compensation and medical, surgical,
<br />hospital, pharmacy, psychological/psychiatric, rehabilitation and nursing expenses/services, or
<br />which anyone claiming by, through or undet• him including, but not limited to, his heirs,
<br />executors, administrators, successors and assigns rhay have or claim to have hereinafter against
<br />City of Lakewood and each and every one, without limitation, of its predecessors, successors,
<br />subsidiazies, affiliates, assignees, divisions, principals, stockholders, officers, directors,
