employees, insurers, agents, attorneys, consultants, or other representatives and their successors
<br />and assigns, without limitation, for any and al] injury or injuries, any and all occupational disease
<br />or diseases, disability, loss or impanment of earning capacity, past, present or future, arising from
<br />or out of or growing out of or in any way connected with or having to do with any injury or any
<br />occupational disease claimed to have been sustained or contracted at a~ay time while Flaintiff-
<br />Appellee, David M. Vet, was an employee ofDefendant-Appellant, City of Lakewood, and
<br />whether said claim or claims at•e for compensations benefits, damages, medtcal, surgical,
<br />hospital, pharmacy, psychiatric/psychological, rehabilitation and nursing expenses and services,
<br />attorney's fees, or expenses of any kind or nature whatsoever, or as the result of or arising from
<br />or growing out of any and all other claims, demands, liabilities, damages, actions and causes of
<br />action under State and/or Federal statutory or common law including, but not limited to, unlawful
<br />discharge, or any claim or cause of action pursuant to R.C. 4123.90, retaliatory discharge,
<br />constructive discharge, an intentional tort action, an alleged violation of a specific safety
<br />requirement, age, race, sex, handicap, national origin or any other form of alleged discrimination,
<br />or violation of the American with Disabilities Act (ADA), the Age Discrimination in
<br />Employment Act (ADLA), Older Workers Benefit Protection Act, the Civil Rights Act of 1964,
<br />the Civil Rights Act of 1991 (42 U.S.C. Suction 2000 et seq.), Sections 1981 through 1988 of
<br />Title 42 of the United States Code, the Employee Retirement Income Security Act of 1974, as
<br />amended, the Occupational Safety and Health Act, as amended, the Family and Medical Leave
<br />Act (FMLA), or any other action arising from or related to the employment relationship.with and
<br />between David M: Vet and City of Lakewood and in full, complete and final satisfaction and
<br />settlement of Claim No. 07-806182 and any and all other numbered or unntunbered claim or
