claims including those which Plaintiff-Appellee, David M. Vet, has, might or could have filed <br />with the Industrial Commission of Ohio, with the Ohio Bureau of Workers' Compensation <br />and/or the Administrator thereof, with any agency of any other state similar to the Industrial <br />Commission of Ohio or the Ohio Bureau of Workers' Compensation or with or against the <br />within-named City ofLakewood <br />It is further agreed and understood that David M. Vet, voluntarily agrees to release and <br />waive any tight to further employment or future employment with City of Lakewood and has <br />hereby relinquished said rights without coercion or duress and, further, David M. Vet, agrees not <br />to seek employment or re-employment with City of Lakewood at any time in the future. If David <br />M. Vet does seek employment or re-employment with City of Lakewood, David M. Vet agrees <br />and understands that City of Lakewood will deny him employment or re-employment. Should <br />David M. Vet apply, he agrees and recognizes that City of Lakewood shall have no obligation to <br />consider his application given the terms of this paragraph. <br />David M. Vet agrees that the facts, terms and amount of this Settlement Agreetent and <br />Release shall be completely confiidential and the nature and ternts hereof shall not be disclosed to <br />any publication or to any other person or parry other than to his spouse, if any, or within any <br />judicial proceeding where disclosure of this Settlerpent Agreement and Release is ordered by the <br />court, or where disclosure is compelled by law and/or in conjunction with the enforcement of this <br />Settlement Agreement and Release and, further, David M. Vet agrees that the facts, terms and <br />amount of this Settlement Agreement and Release shall not be disclosed, discussed or revealed to <br />any other person, firm, corporation or publication, except for his attorneys, health care providers <br />and personal financial, tax advisors or accountants, and then only if each and every such person, <br />-5- <br />
