Exhibit A <br />2012 Senior Farmers' Market Nutrition Program <br />Distribution Agent Agreement between <br />Western Reserve Area Agency on Aging <br />And- <br />Lakewood Division of Aging <br />1. This agreement is between the Western Reserve Area Agency on Aging <br />(WRAAA) located at 925 Euclid Ave, Suite 600, Cleveland; Ohio 44115 and the <br />Lakewood Divtsion of Aging (Distribution Agent) for the distribution of Senior <br />Farmers' Market Nutrition Program coupon sets to eligible participants at the <br />following location: <br />Lakewood Division of Aging, 16024 Madison Avenue, Lakewood, 44107 <br />2. The WRAAA in accordance with guidelines set forth by the Ohio Department of <br />Aging, will provide the Distribution Agent with coupon sets to be distributed as set <br />forth in the guidelines outlined in the Distribution Agent Manual. These coupons <br />will be distributed to each site as follows: <br />Lakewood Division of Aging will receive 50 coupon sets valued at $2,500.00 <br />The Distribution Agerit agrees: <br />a, to distribute coupons on a first come, first serve basis <br />b. to accept applications from participants who reside in a WRAAA defined <br />service area based on the local area postal zip codes stated below: <br />- ~ 44107 <br />c. Seniors seeking coupons, not presently served at the senior center, and <br />that live in a zip code outside this Distribution Agent's zip code, will be <br />directed to the Distribution Agent in that senior's zip-code. <br />d. to work with local senior housing facilities for application distribution. <br />e. not to reserve applications or coupons for any individual or groups, except <br />for proxies who must go to the senior participant's residence to have the <br />_ participant sign the application. <br />4. The Distribution Agent will take full responsibility for the Senior Farmers' Market <br />Nutrition Program (SFMNP) Coupons and their monetary value upon receipt of <br />coupons. <br />a.
