Summary of recommendations: <br /> 1. Live Well Lakewood Workgroup Phase 2 becomes Phase 2 of the Resiliency Task Force with a <br /> deeper focus on Health & Wellbeing. <br /> 2. Budget$250,000.00 to be allocated to the work of Live Well Lakewood Phase 2. <br /> 3. Live Well Lakewood Phase 1 Workgroup will nominate an Advisory Committee from its <br /> members to work with the Resiliency Task Force staff member to: <br /> a) Solicit, interview and recommend a facilitator to guide the work of the Resiliency Task <br /> Force Phase 2 focusing on Health & Wellbeing <br /> b) Solicit, interview and recommend an Academic Fellow to work with the Resiliency <br /> Task Force Phase 2 to conduct a Community Health Needs Assessment for Lakewood. <br /> c) Solicit applications and recommend for appointment by the Mayor and City Council, <br /> the Phase 2 Resiliency Task Force members to focus on Health &Wellbeing <br /> 4. Live Well Lakewood Workgroup Phase 2 to focus on: <br /> a) the development of a Lakewood Community Health Needs Assessment which will <br /> include the engagement of all Lakewood populations and will seek IRB approval. <br /> b) defining the components of a community action plan for health and wellbeing <br /> c) outreach and interviewing potential entities, organizations and businesses that should <br /> be considered as initial stakeholders for the future crass-sector health collaboration <br /> with the goal to locate drivers for this initiative. <br /> 5. Initiate Cross-Sector Collaboration—Phase 3 <br /> Resiliency Task Force(RTF) Live Well Lakewood Work Group (LWL) <br /> • Convened April 2018 Volunteer group convened in June 2019 <br /> • Members appointed by City Council& • Purpose is to think critically about how <br /> Ui the Mayor;Alex Harnocz heads the task Lakewood can effectively impact the <br /> force and is paid City employee health and wellness of all of its citizens <br /> • Purpose was to understand current and • Goal was to identify the path forward to <br /> emerging forces that can impact recognizing the best way to impact the <br /> Uj Lakewood and the wellbeing of its health and wellness of all Lakewood <br /> citizens citizens <br /> • Health and Wellbeing specifically LWL developed recommendations and is <br /> identified as one of six priorities seeking funds to of$250,000 to be <br /> Original RTF members will complete their directed to the right entity to oversee <br /> • phase two <br /> term in November 2019, new RTF <br /> members to be identified in Phase 2 <br />
