taW44l Rd-$125. 0 per o:' • for single. <br />. <br />The ` 100%" and the "90/10" Summary of Benefits and Coverage for 2017, including prescription drug <br />coverage, as well as a chart of approved plan design changes to be put into effect during the life of this <br />contract are attached hereto as Appendix C. <br />The Employer shall establish an Insurance Committee of one (1) to three (3) representatives from each of <br />the City's bargaining units, if they choose to be represented and a minimum of one (1) representative of <br />the Employer. The Committee shall meet at least once a month for the purpose of exploring cost saving <br />measures and/or alternative health plans. The Committee shall make recommendations regarding health <br />care coverage and such recommendations shall be presented to each bargaining unit as well as to the City <br />Administration. This Committee shall be administered pursuant to Appendix F, attached hereto. <br />17.03 Life Insurance <br />The City shall provide a policy of life insurance in the amount of $25,000.00 to every employee of the <br />Division of Fire who shall have the right to convert said policy at the rates established by the insurance <br />carrier upon the employee's termination. The insurance shall be written by a recognized insurance carrier <br />and all premium costs shall be borne by the City. <br />17.04 Liability Insurance <br />Subject to the limits of the policy, whenever an employee is on duly, insurance coverage shall be provided <br />when said employee of the Division of Fire is operating any city owned vehicle in the performance of <br />assigned duties to protect the employee fully against any and all claims arising out of the authorized oper- <br />ation of said city vehicle. This insurance must cover all claims for property damage and personal injury, <br />and shall be the sole responsibility of the City to provide and maintain. Additionally, the City will provide <br />full legal defense to any employee in any legal action arising out of the authorized operation of said city <br />vehicle. <br />17.05 Accidental Death and Dismemberment Policy <br />The City shall provide an Accidental Death and Dismemberment (AD&D) Insurance Policy for all bargain- <br />ing unit employees in the amount of $125,000.00. <br />ARTICLE 18 <br />DISCRIMINATION <br />18.01 Both the City and the LPA recognize their respective responsibilities under the Federal and State <br />Civil Rights Law, Fair Employment Practice Acts, and other similar constitutional and statutory require- <br />ments. Therefore, both the City and the LPA hereby affirm their commitments, legal and moral, not to <br />discriminate, in any manner, relating to employment on the basis of race, color, creed, national origin, sex, <br />age, disability, gender identity/expression, genetic information, military status, veteran status, sexual <br />13 <br />
