#¨³¸ ®¥ , ª¤¶®®£ ȟ #®¬¬´¨³¸ (¤ «³§ !¢³¨® 4¤ ¬ ȟ )³¤±¨¬ 0±®¦±¤²² 2¤¯®±³ Ȩ3¯±¨¦ ȟ & «« ΑΏΑΒȩ Background: 2022 CHNA& Committee Establishment Lakewood, Ohio In 2021, the City of Lakewood initiated a 16-month contract with the Center for Community Solutions (CCS) to develop a comprehensive Community Health Needs Assessment (CHNA)and supporting Action Plan to inform stakeholders and residents on the health of the Community Health Action Team community using thesocial determinants of health framework. The CHNA process was overseen by a Steering Committee with )³¤±¨¬ 0±®¦±¤²² 2¤¯®±³ Ȩ3¯±¨¦ ȟ & «« ΕΓΕΖȩ representatives from the City of Lakewood Planning and Human Services departments, the Healthy Lakewood Foundation, andthe ThreeArches Foundation. Additionally, an Advisory Committee of over 20 community leaders was leveraged to inform the methodology, outreach, surveys, and priorities of theCHNAprocess. The assessment was completed and introduced in September 2022, and after soliciting community feedback and ideas, an Action Plan The Action Plan was developed through a charrette model of engagement that allowed community £ ®¥¥¤±²´¦¦¤²³¨®²Ȩȏ¨³¤±µ¤³¨®²Ȑȩ³® address various issues. The charrettes were conducted both online and through in person events - garnering 168 reactions from 39 individuals.A total of 54 interventions were made by the participating community members. The Steering Committee and Advisory Committee members then worked with CCS to create an evaluation tool using a process that established both an evaluation criterionand a weighting systemof the determined valuefor each criterion which resulted in a total score for each of the 54 suggested interventions. Members of both committees then evaluated each of theinterventionsthrough this methodology and atotalcomposite score for each intervention was calculated. Along with these scored interventions, the Action Plan also offered three key recommendations to advance the assessment and action plan from reports to interactive tools to determine priorities, engage stakeholders,and advocatefor policies, funding, and programming.
