#¨³¸ ®¥ , ª¤¶®®£ ȟ #®¬¬´­¨³¸ (¤ «³§ !¢³¨®­ 4¤ ¬ ȟ )­³¤±¨¬ 0±®¦±¤²² 2¤¯®±³ Ȩ3¯±¨­¦ ȟ & «« ΑΏΑΒȩ <br />Background: <br />2022 CHNA& Committee Establishment <br />Lakewood, Ohio <br />In 2021, the City of Lakewood initiated a 16-month contract with the <br />Center for Community Solutions (CCS) to develop a comprehensive <br />Community Health Needs Assessment (CHNA)and supporting <br />Action Plan to inform stakeholders and residents on the health of the <br />Community Health Action Team <br />community using thesocial determinants of health framework. The <br />CHNA process was overseen by a Steering Committee with <br />)­³¤±¨¬ 0±®¦±¤²² 2¤¯®±³ Ȩ3¯±¨­¦ ȟ & «« ΕΓΕΖȩ <br />representatives from the City of Lakewood Planning and Human <br />Services departments, the Healthy Lakewood Foundation, andthe <br />ThreeArches Foundation. Additionally, an Advisory Committee of over <br />20 community leaders was leveraged to inform the methodology, <br />outreach, surveys, and priorities of theCHNAprocess. <br />The assessment was completed and introduced in September 2022, <br />and after soliciting community feedback and ideas, an Action Plan <br />The Action Plan was developed <br />through a charrette model of engagement that allowed community <br /> ­£ ®¥¥¤±²´¦¦¤²³¨®­²Ȩȏ¨­³¤±µ¤­³¨®­²Ȑȩ³® <br />address various issues. The charrettes <br />were conducted both online and through in person events - garnering 168 reactions from 39 <br />individuals.A total of 54 interventions were made by the participating community members. <br />The Steering Committee and Advisory Committee members then <br />worked with CCS to create an evaluation tool using a <br />process that established both an <br />evaluation criterionand a weighting systemof the determined <br />valuefor each criterion which resulted in a total score for each <br />of the 54 suggested interventions. Members of both committees <br />then evaluated each of theinterventionsthrough this <br />methodology and atotalcomposite score for each intervention <br />was calculated. <br />Along with these scored interventions, the Action Plan also <br />offered three key recommendations to advance the assessment <br />and action plan from reports to interactive tools to determine <br />priorities, engage stakeholders,and advocatefor policies, <br />funding, and programming.
