RESOLUTION NO. 8304-08A BY: Antonio, Bullock, Butler, Dever, <br />Madigan, Powers, Summers. <br />A RESOLUTION to take effect immediately provided it receives the affirmative <br />vote of at least five (5) members elected to Council otherwise, it shall take effect and be <br />in force after the earliest possible period allowed by law, to amend Resolution NO. <br />8304-08, adopted October 20, 2008 to allow the Mayor, or his designee, on behalf of <br />the City, to enter into a Partnership Agreement with The Community Care Network and <br />Positive Education Program to enable the City of Lakewood to receive the services of a <br />Tapestry System of Care Parent Advocate and a Positive Education Program Tapestry <br />Care Coordinator for the period of November 1, 2008 to October 31, 2009 and to <br />receive reimbursement for overhead costs associated with those position in an amount <br />not to exceed $24,500 from The Community Care Network. <br />WHEREAS,. the Cuyahoga County Board of County Commissioners (BOCC) <br />accepted a federal Substance Abuse and Mental Health Services (SAMHSA) System of <br />Care grant called TAPESTRY and established the Cuyahoga Tapestry System of Care <br />(CTSOC) office to administer the TAPESTRY grant and to integrate all Cuyahoga <br />County System of Care Initiatives for children and families; and <br />WHEREAS, The Community Care Network has entered into a contract with the <br />BOCC to receive funding for the (CTSOC) Parent Advocate Program in Lakewood; and <br />WHEREAS, The Community Care Network will be responsible for providing a <br />Parent Advocate for the City of Lakewood including all financial obligations including <br />salary, benefits, mileage, travel and professional registrations; and <br />WHEREAS, currently the Positive Education Program employs six Tapestry Care <br />Coordinators (PEP Care Coordinators) who work out of one central location. In an effort <br />to bring services out into the communities that they serve, one employee will be <br />relocated to the City of Lakewood, Division of Youth's offices; and <br />WHEREAS, the City of Lakewood will receive $24,500.00 in compensation for <br />providing the Parent Advocate and PEP Care Coordinator office space, equipment, <br />supplies, supervision and overhead costs; and <br />WHEREAS, the Parent Advocate and PEP Coordinator will be an integral <br />member of the Lakewood Family to Family System of Care team, providing support for <br />families and advocates for them with other CTSOC and other public service providers; <br />and <br />WHEREAS, this Council by a vote of at least five (5) members elected thereto <br />determines that this resolution is an emergency measure, and that this resolution shall <br />take effect at the earliest date possible as set forth in ARTICLE III, SECTIONS 10 and <br />13 of the SECOND AMENDED CHARTER OF THE City of Lakewood, and that it is <br />necessary for the immediate preservation of the public property, health and safety, and <br />
