<br />The undersigned, who resides at '

<br />which is hereby acknowledged, does voluntarily and knowingly execute
<br />this Release with the express,intention of effecting the extinguish-
<br />ment of obligations as hereinafter designated.
<br />The undersigned, with the intention of binding himself, his heirs,
<br />exccutors, administrators, and assigns, does hereby expressly release .
<br />and discharge Mayfield Village, an Ohio Municipal Corporation, its
<br />officials, officers, agerits and employees, from a11 claims, demands,
<br />actions, judgments, and executions wh'ich the undersigned ever had, or
<br />now has, or may have, or which the undersigned's heirs, executors,
<br />administrators of assigns may have, or claim to have, against Mayfield
<br />Village, its officials, officers, agents and employees, created by or
<br />arising out of the use of a(n) (insert name of tool)

<br />Mayfield Village by the undersigned pursuant to the Tool and Equipment
<br />Lending Program established by Mayfield Village, Ordinance No. 82-88.
<br />The undersigned further elects to, and does, assume alI risk for claims
<br />heretofore or hereafter arising, known or unknown, arising from the
<br />subject of this Release, in favor of the undersigned, his heirs,
<br />executors, administrators, successors or assigns and hereby knowingly
<br />and voluntarily expressly releases Mayfield Village, its officials,
<br />officers, agents and employees, from all liability for claims arising
<br />from the subject of this Release.
<br />By executing this Release the undersigned represents that he/she has
<br />knowledge of how to use.the borrowed tool(s) or will obtain requisite
<br />knowledge prior to using the tool(s).
<br />The undersigned has read this Release and understands all of its terms
<br />and executes it voluntarily and with full knowledge of its significance.
