M.V. ORDINANCE N0. 85-1 Page -10- shall require an employee to furnish a satisf actory written signed statement to justify the use of sick leave. If absence due to illness exceeds three (3) days, a statement certifying the nature of the illness from a licensed physician shall be required to justify the use of sick leave. Falsification of either a written statement or a physician's certificate shall be grounds for disciplinary action including aismissal. No sick leave may be granted upon or after retirement or termination of employment. 2. Any permanent full-time employee who has an accumulation of forty-five (45) days unused sick leave will be compensated at the end of the year for the number of hours not taken up to a maximum of five work days. Accumulation of sick leave shall excluae hours for which compensation has been paid. 3. Upon retirement all full-time employees shall be entitled to receive credit and payment not to exceed thirty (30) calendar days in lieu of unused accumulated sick leave, provided that they have been employed by the Village f or ten (10) years or more. For purposes of this section, an employee shall be considered to have retired when such employee takes action that is inconsistent with any reasonable intent to continue employment with the municipality. 4. If a full-time employee is scheduled to work on a holiday enumerated in Section 6(a), and cannot work due to illness, that day shall be considered a holiday day off and shall not be counted or compensated as a sick day. H. BEREAV EMENT LEAV E In the event of death in the employee's immediate family, he shall be entitled to three (3) days off, or in the case of a member of the Fire Department unc3er the rank of Chief, one twenty-four (24) hour shift. The "IMMEDIATE FAMILY" shall include the spouse, children, parents, brothers, sisters, spouse's parents and grandparents. Should an exception to the af orementioned apply, such exception to be authorized by the Mayor. This leave shall not be charged to any other leave provision provided by the Ordinance. I. HOSPITALIZATION AND MEDICAL COVERAGE. 1. The municipality shall pay the premiums for all permanent full-time employees insured under a contract of hospitalization and medical coverage selected by Mayfield Village. 2. Permanent full-time employees of the municipality not presently insured and new permanent full-time employees of the municipality may join the plan at such intervals as may be authorized by the municipality's medical insurance carrier, subject to the approval of the Mayor and Finance Director.
