-4- CUYAHOGA REGIO%AL INFORMATION SYSTEA4 AHENDMENT TO SERVICE.AGREEt9EP7T OF FEBRUARY 18, 1982 THE AMENDYNG OF A SERVICE AGREEMENT BY AND BETZ4EEN THE BOARD OF COUNTY COMMISSIONERS, COUNTY OF CUYAHOGA,OH2O AND THE CUYAHOGR REGIONAL INFORAiATION SYSTE24 BOARD OF ADVISORS (hereinafter called the COUNTY) AND THE MAYFIELD VILLA6E POLICE DEPT. (hereinafter called the USER). WITNESSETH THAT: WHEREAS, the COUNTY is operating the Cuyahoga Regional Information System (hereinafter called the CRYS), A Police and Criminal Justice oriented information system, and WHEREAS, the,USER has previously participated in the CRIS and wishes to continue participation in the CRIS, and NOW, THEREEORE, it is agreed that the Service Agreement by and between the COUNTY, the CRIS, and User, is hereby amended to extend the service from January 1, 1990 until December 31, 1490. The Schedule of Charges for the provision of this service during the period January 1, 1990 to December 31, 1990 shall be as follows: The following schedule of charges shall be used hy the COUNTY in billing the USER for services provided by the County to the USER under this Agreement: - SERVICE $ /A90NTHLY UNIT RATE CATEGORY RATE General Center $ 153.59 Local Network 104.75 Master Terminal 131.19 Computer Resource 704.23 "CRIS" Application 31.24 "HAZMIS" Application 0.00 TOTAL PER MONTH $1,130.00 The One-Thousand-One-Hundred and Thirty dollars ($1,130.00) per month charge shall begin upon the COUNTY making available the CRIS service, as defined by the CRIS Board of Advisors, to the USER on January 1, 1990. Services provided by the COUNTY to the USER which arekoutside the normal scope of services as defined by the CRIS Board of Advisors shall be billed to the USER by the COUNTY at the actual cost to the COUNTY of providing the requested services. All other provisions of the original Agreement shall remain in full force and effect. 0
