' ."EXHIBIT A" <br />~h~b~+ A <br />CUYAHOGA COUNTY PLAN OF COOPERATION REGARDING <br />CHILD ABUSE AND NEGLECT <br />AUTHORITY As required by section 2151.421 of the Ohio Revised Code, <br />this plan is entered into by the Cuyahoga County Department <br />of Human Services, Division of Social Services (CCDHS-SS) <br />and i rtrn <br />Law Enforcement Agency) <br />PURPOSE To ensure the prompt reporting of incidents of suspected <br />child abuse and neglect and to provide for cooperation <br />among the agents who must investigate such reports so that <br />the children and their families may receive needed services <br />and protection. ' <br />PAdTiCIPATION OF THE PARTIES <br />EMERGEiVCY SITUATIONS <br />Definition: "Emergency situations" are those situations in which there <br />are reasonable grounds to believe that the child is suffering from illness <br />`or injury and is not receiving proper care, or is in immediate danger from <br />his/her surroundings, and that removal may be necessary. <br />1. The Cuyahoga County Department of Human Services, Division of Social <br />Services is located at 3955 Euclid Avenue, Cleveland, Uhio, and is <br />open Monday through Friday from 8:15 a.m. to 5:00 p.m. Situations <br />which, in the judgment of peace officers, require immediate protec- <br />tive action can be reported by telephone to 431-4500 or in person. If <br />removal of the child(ren) is warranted, the peace officers will bring <br />the child(ren) to 3955 Euclid Avenue for placement. <br />2. Any situations which occur during the weekends and between the hours <br />of 5:00 p. m, and 8:15 a.m., Monday through Friday and where, in the <br />judgment of peace officers, children are found to be without proper <br />adult protection or supervision or are otherwise seriously neglected, <br />abused, and in need of immediate placement, may transport the child ur <br />children to the Bessie Benner i4etzenbaum Children's Center at 3343 <br />Community College Avenue, Cleveland, Ohio which is the twenty-four <br />(24) hour child care facility operated by the Cuyahoga County Human <br />Services Department, Division of Social Services. In these <br />situations, a report should be telephoned to agency personnel on duty <br />during these hours at 69ti-KIDS. A written report is to follow phone <br />referrals, addressed-to the Intake Department, Cuyahoga County Depart- <br />ment of Human Services, 3955 Euclid Avenue, Cleveland, Ohio. <br />3. Children who appear to have been abused or otherwise need immediate <br />medical attention should be taken directly to the nearest hospital. <br />The same reporting procedure is to be followed. <br />
