CROSS REFERRAL PROCEDURES FOR EXCHANGE OF INFORMATION BETWEEN <br />SUBSCRIBERS. <br />Upon receipt of a report indicating child abuse or neglect, such <br />reports will be exchanged between Cuyahoga County Department of Human <br />Services, Social Services and the receiving law enforcement agency as <br />required by Ohio Revised Code section '2151.421 and O.A.C. Rule 5101.2- <br />35-03. IJhere a report is received by the Human Services Department <br />involving the deatn, current sexual abuse,~or injury requiring hospi- <br />talization of a child, such information will be immediately telephoned <br />to the appropriate Law enfurcement agency which For the 461-1234 <br />Mayfield Village Police Department is 461-2210 <br />Law EnForcement Agency Telepnone v <br />A written report of any investigation undertaken by the agency subse- <br />quent to the death of a child will be provided to the law enforcement <br />agency. <br />Upon completion of an investigation by file Human Services Department <br />or law enforcement agencies, a caritten report thereof will be <br />exchanged. <br />Such exchange of information applies to all situations described in <br />this plan which require investigation by the Human Services Depart- <br />ment and/or law enforcement agencies. Referrals will be made in a <br />timely Fashion to enable a response to be made according to file <br />prescribed procedures for emergency and non-emergency situations. <br />Written reports of investigations will be exchanged within 45 days <br />after the initial report. <br />lU. The Law enforcement agencies of Cuyahoga County which are signatories <br />[o this agreement caul receive and respond to reports or cases <br />involving individuals who aid, abet, induce, cause, encourage or con- <br />tribute to a child or a ward of the Juvenile Court: <br />- becoming a dependent, neglected, or abused child, or <br />- becoming an unruly or delinquent child, or <br />- leaving the custody of any person, department or public or <br />private institution without the legal consent of that <br />person, department, or institution. <br />
