ALLAN, R.S., M.P.H. <br />HEALTH COMMISSIONER <br />www.ccbh.net <br />5550 VENTURE DRIVE PARMA, OHIO 44130 (216) 201-2000 <br />March 8, 2004 <br />Mayfield Village <br />RE: Phase II Storm Water <br />Per your request, the Cuyahoga County Board of Health is glad to submit to you a proposal for Phase II Storm <br />Water related activities. The educational outreach programs that we can offer and assist your community <br />would not be a charged service. However, the other activities that we may perform will be. <br />The activities that we can provide your community but will charge for that service include: <br />To survey the Mayfield Village's MS4 outfall locations, digital photograph and GPS these locations would <br />cost $3,500.00. This would include walking of all surface waters within the city and locating any MS4 <br />outfall in these bodies of water. The City would receive a binder with all outfall locations (see attached <br />example of outfall inventory report that would be in binder -each outfall would have its own report), a <br />copy of the database with the city's outfall information included, and a map of the city with all MS4 <br />outfall locations identified. <br />2. To visually inspect these locations during dry weather conditions for the observance of dry weather flow <br />would cost $800.00 per year. <br />3. To sample up to 25 outfall locations for fecal coliform the total cost would be $1,000.00.
