CENTER/W~XTE ROAD WIDENING <br />CONSTRUCTION ADN(Xr(XSTgATION AND ENCINEE~t~11iG <br />URS CORPORATION/MAYFIELD VILLAGE _ <br />AGREEMENT DATED FEBRUARY 3, I998 <br />MODIFICATIQN 1~T0. 14-Dated June 2, 2004 <br />This Work Order Task is being performed as an extension of the Professional Engineering Services <br />Agreement between Mayfield Village and UR8 Cvrprl'rutiun (URS'), formerly k~trn-~n as Yl'RS <br />Greiner, dated February 3, 1998. AlI terms and conditions stated therein are incorporated into this <br />Work Order by reference. The MOD No. ] 0 modifications to the base document, which was <br />previously approved by the Clit;nt, are annotated by bold italics font. Resident inspection services <br />were moved to Work Order No. 7 and incorporated therein as part of Modification No. 1. <br />SCOPE OF SERVICES <br />This Work Order provides for bidding and construction administration services during the <br />construction of the widening and rcconstrucLion of S.O.M. Center/White Roads under one (1) <br />construction contract. The contract for the construction of S.O.M. Center Road steal I be twenty-four <br />(24) months induration. S.O.M. Center Road would be widened to five (5) lanes for an estimated <br />Ic'ngth of twelve thousand seven hundred (12,700) lineal foot. White Rvad would be widened to <br />three (3) lames for an estimated length of twelve hundred (1,200) lineal foot 'fhe design, plan <br />preparation, and construction phase services would be performed to be consistent with the flew <br />ODOT Lacal Public Agency Policy. <br />URS will provide the following specific consulting services in assisting the Client during the bidding <br />and construction phases of the project: <br />Basic Services During Bidding Phsrse: The Bidding Phase will commence upon the approval by <br />the Client of the Contract Drawings and Project Manual and will terminate upon URS furnishing the <br />Client with an engineering recommendation concerning the awarding of the construction contract. <br />URS' services during the Bidding Phase shall consist of the following: . <br />I . URS shall issue; on bChalf of the Client up to 40 sets of Drawings and Project Manuals tv <br />]n[erested COnCi'aCCorS atld cqui,pment and materials suppliers upon their request and maintain <br />records of the bidders list. URS shall be entitled to charge the Contractor's, suppliers and <br />other interested parties a reasonable fee l:or purchase of the Drawings and Project Manuals <br />and will retaui saeh fee as additional compensation. <br />2. URS shall place copies of the Drawings and Project Manual on view in URS' Cleveland <br />office, F.W.
