ORDINANCE 95-78 <br />I. Bereavement Leave <br />In the event of death in the employee's immediate family, he shall be entitled to three (3) days off, or in the case <br />of a member of the Fire Department under the rank of Chief, one (1) twenty-four (24);hour shift. The "immediate <br />family" shall include the spouse, children, parents, brothers, sisters, grandparents and spouse's parents. Should an <br />exception to the aforementioned apply, such exception to be authorized by the Mayor. An employees use of this <br />leave while on vacation, holiday, or sick leave, shall result in the employee not being charged for such vacation, <br />holiday or sick leave. <br />J. Hospitalization and Medical Coverage <br />1. The Municipality shall pay the premiums for all permanent fall-time employees insured under a contract of <br />hospitalization and medical coverage selected by Mayfield Village. <br />2. Permanent full-time employees of the Municipality not presently insured and new permanent full-time <br />employees of the Municipality may join the plan at such intervals as may be authorized by the Municipality's <br />medical inc~irance carrier, subject to the approval of the Mayor and Finance Director. <br />3. Any permanent full-time employee who is otherwise eligible to be covered under a contract of hospitalization <br />and medical coverage but voluntarily withdraws from the above-referenced in~,rance coverage shall be paid the <br />amount of One Thousand Dollars ($1,000.00) for each year the employee is employed by Mayfield Village but <br />does not request coverage. For the time period covered by this ordinance, dental coverage will not be considered <br />as part of the hospitalization or medical coverage described above. The payment authorized herein shall be made <br />in equal quarterly installments. This payment will not be made to any person who is otherwise covered under the <br />Mayfield Village policy through a Village employed spouse or other family member. (Effective in 1994). If an <br />employee is eligible under the terms of this paragraph but has worked less than one full year, the benefit <br />described herein will be prorated on a monthly basis beginning the first month in which the employee would have <br />received the benefit under the plan provided by the Village. <br />4. Sections 1, 2 and 3 above shall be reviewed at the end of fiscal year 1996 and reconsidered as to all aspects. <br />K. Overtime <br />1. Overtime will be paid in accordance with the Fair Labor Standards Act. as amended. <br />2. All overtime shall be approved in advance by the Mayor unless there exists an emergency that threatens the <br />health, safety or welfare of the Municipality. <br />3. If an employee is scheduled for training or seminars while on vacation or holiday, the employee shall be paid <br />regular time and restored that vacation or holiday time lost. No overtime will be paid in this situation . <br />4. Time changes relevant to day light savings shall not entitle the Village to dock pay nor to pay additional time <br />on those days, as same will balance out. <br />L. Jurv Dutv Travel. Conference and Other Expense <br />1. Internal Revenue Service permissible payment as determined by Finance Director per mile, is hereby allowed to <br />reimburse employees of the City who are required to make trips on City business in their own automobiles, other <br />13 WAGE ORDINANCE <br />
