Upon completion of the entry covered by the permit, and after all entrants have <br />exited the permit space, the individual authorizing the entry shall cancel the <br />permit. <br />12. Review entry operations when the Vfflagbas reason to believe that the measures <br />taken under the permit space program may not protect employees and revise the <br />program to correct deficiencies found to exist before subsequent entries are <br />authorized. <br />13. Thed]]-Tshall establish and implement the means, procedures and practices by <br />which the pernut space can be entered safely. By such signs and training, unauthorized personnel will be prevented from <br />entering permitted confined space area. <br />E. Personnel Training <br />1.
