Mayfield Village <br />ADI)ENDUM B <br />SERVICES PROVIDED <br />The softwaze delivery and installation consists of the following three major phases: <br />PHASE I- DELIVERY OF SOFTWARE PACKAGES <br />Creative Microsystems, Inc. <br />Delivery of software shall mean the delivery of the software to the customer site and the act of loading the software programs <br />on the customer's hardware. Software fees will be billed and payable upon the receipt of invoice. <br />PHASE II - TRAINING CLASSES <br />Training as provided for on Addendum A. It is recommended that all training be held at the CMI Training Center in Dayton. <br />The value of each class is currently $150 per day per person. Half day classes have a value of $75.00 per person. <br />PHASE III - INSTALLATION <br />Installation of software represents the efforts associated with on-site setup, implementation, assistance, minor software <br />modifications and installation support. The cost for Software Support Services will be billed at the start of the installation process <br />and is payable upon receipt of invoice. <br />Installation does not include conversion of data from existing systems. Such services can be handled on a time and material <br />basis. <br />Services not covered under the terms of this agreement will be billed at the CMI hourly billing rate applicable at the time of <br />billing. Conversion of data from the customer's existing system is the direct responsibility of the customer. CMI will assist in <br />this process to the extent it deems appropriate and at a cost to be determined at the time any conversion effort is defined. <br />Once the installation process has been completed, on-site support will be billed on an "as needed" basis at the hourly rate in effect <br />at that time. The cost for such ongoing support is not included on Addendum A and should be handled by means of an annual <br />support services purchase order. <br />PHASE IV - ONGOING SOFTWARE SUPPORT <br />Software support includes telephone and modem support. Our technical support staff near Dayton, Ohio will answer questions <br />and in some cases will log onto your system via modem to aclueve problem resolution. If you require on-site assistance, it will <br />be provided on an hourly basis at the CMI support rate which is currendy $55 per hour. <br />8
