I. Bereavement Leave <br />In the event of death in the employee's immediate family, he shall be <br />entitled to three (3) days off, or in the case of a member of the fire <br />Department under the rank of Chief, one (1) twenty-four (24) hour shift. The <br />"immediate family" shall include the spouse, children, parents, brothers, <br />sisters, spouse's parents and grandparents. Should an exception to the <br />aforementioned apply, such exception to be authorized by the Mayor. An <br />employees use of this leave while on vacation, holiday, or sick leave, shall <br />result in the employee not being charged for such vacation, holiday or sick <br />leave. <br />J. Hospitalization and Medical Coveracie <br />1. The Municipality shall pay the premiums for all permanent full-time <br />employees insured under a contract of hospitalization and medical coverage <br />selected by Mayfield Village. <br />2. Permanent full-time employees of the Municipality not presently <br />insured and new permanent full-time employees of the Municipality may join <br />the plan at such intervals as may be authorized by the Municipality's medical <br />insurance carrier, subject to the approval of the Mayor and Finance Director. <br />3. Any permanent full-time employee who is otherwise eligible to be <br />covered under a contract of hospitalization and medical coverage but <br />voluntarily withdraws from the above-referenced insurance coverage shall be <br />paid the amount of One Thousand Dollars ($1,000.00) for each year the <br />employee is employed by Mayfield Village but does not request coverage. The <br />payment authorized herein shall be made in equal quarterly installments. <br />This payment will not be made to any person who is otherwise covered under <br />the Mayfield Village policy through a Village employed spouse or other family <br />member. (Effective in 1994). <br />4. Sections 1, 2 and 3 above shall be reviewed at the end of fiscal <br />year 1993 and reconsidered as to all aspects. <br />K. Overtime <br />1. Overtime will be paid in accordance with the Fair Labor Standards <br />Act, as amended. <br />2. All overtime shall be approved in advance by the Mayor unless there <br />exists an emergency that threatens the health, safety or welfare of the <br />Municipality. <br />3. If an employee is scheduled-for training or seminars while on <br />vacation or holiday, the employee shall be paid regular time and restored <br />that vacation or holiday time lost. No overtime will be paid in this <br />situation. <br />4. Time changes relevant to day light savings shall not entitle the <br />Village to dock pay nor to pay additional time on those days, as same will <br />balance out. <br />12 <br />
