ARTICLE 28 <br />INSURANCES <br />Section 28.1. During the term of this Agreement, the Village may, where economically feasible <br />maintain substantially similar levels of inedical and dental service benefits including prescription <br />riders and life insurance. ' <br />Section 28.2. It is understood and agreed that the Village will establish a"Health, Care Task <br />Force" comprised of appropriate representatives of the Village administration and Village <br />employees, including representatives of the F.O.P. The Task Force will be expected to study the <br />problems of health care and health care cost containment and then formulate recommendations <br />which will be considered by the Village. <br />Section 28.3. It is understood and agreed that members of the collective bargaining unit will not <br />make contributions toward the cost of the health care plan for the years 2006, 2007 or 2008. <br />Effective January l, 2009, employee contributions shall be Fifty Dollars ($50.00) per month per <br />member for family coverage and Twenty-Five Dollars ($25.00) per month per member for single <br />coverage, which shall be deducted from each employee's pay through automatic payroll <br />deduction. <br />The Village, for the term of this agreement, will continue to provide prescription drug coverage <br />with an eight dollar ($8.00) deductible for Generic, a fifteen dollar ($15.00) deductible for <br />Name Brand and a twenty-five dollar ($25.00) deductible for Formulary. <br />ARTICLE 29 <br />INJURYLEAVE <br />Section 29.1. When an employee becomes injured or disabled as a result of an event arising out of <br />and in the course of bona fide police work as determined by the Village, so as to be physically unfit <br />for duty, the employee sha11 be granted a special leave of absence with pay by the Village beginning <br />with the first working day of such disability. In order to be eligible for the special on the job injury <br />leave, as provided in the article, the employee's disability must be evidenced by a certificate of a <br />physician who examined the employee. Special on the job injury leave shall not be granted to <br />employees who incur injuries of a routine nature or to employees while in the employment of <br />another person or entity. <br />6/1/2006 Page 25 of 36
