ORDINANCE NO. 2006-36-PAGE 11 <br />3) Married couples employed by the Village who request leave under (D) <br />(1), (2), or (3), may have their leave time reduced by the Village to an aggregate <br />of twelve (12) weeks. <br />4) The medical benefits provided to an employee who requests and is <br />granted a medical leave of absence under Leave of Absence Section (D) herein, <br />will continue for the duration of the leave, to a maximum of twelve (12) weeks, <br />at the same level of coverage and on the same terms and conditions in effect <br />when the employee was working. <br />5) Employees are required to substitute any accumulated sick, vacation <br />and personal comp time for the duration of their family or medical leave. <br />D. NOTICE <br />1) If the necessity of family or medical leave is foreseeable, the employee <br />is required to provide at least thirty (30) days advance notice. <br />2) If a foreseeable family or medical leave is predicated upon a serious <br />health condition, the employee must make a reasonable effort to schedule <br />treatment so as not to unduly disrupt the Village's operations. <br />3) If the family or medical leave necessitates leaving within fewer than <br />thirty (30) days, the employee is required to provide notice to the Village as <br />soon as is practicable. <br />4) All requests for leave under this Act should be made in writing to the <br />employee's department head or to the Mayor in the case of a department head. <br />The amount of sick leave and vacation time available should be confirmed with <br />the finance department before application is made. <br />E. <br />1) If an employee requests leave because of a serious health <br />condition, the Village may require, and the employee must timely provide, <br />certification of the condition. The certification should state: <br />a.) The commencement date and probable duration of the <br />condition. <br />b.) The medical facts substantiating the condition; and <br />c.) Either that the employee must care for a parent, spouse, or <br />child with a serious health condition or that the employee <br />cannot perform duties due to a serious health condition.
