ORDINANCE NO. 2006-36-PAGE 12 <br />153.12 HOSPITALIZATION AND MEDICAL COVERAGE. <br />(a.) The Municipality shall pay a portion of the premiums for all permanent full-time <br />employees insured under a contract of hospitalization and medical coverage selected by Mayfield <br />Village. <br />(b.) Permanent full-time employees of the Municipality not presently insured and new <br />permanent full-time employees of the Municipality may join the plan at such intervals as may be <br />authorized by the Municipality's medical insurance carrier, subject to the approval of the Mayor <br />and Finance Director. <br />(c.) Any permanent full-time employee who is otherwise eligible to be covered under a <br />contract of hospitalization and medical coverage but voluntarily withdraws from the above- <br />referenced insurance coverage shall be paid the amount equivalent to 40% of the premium paid <br />by the Village for the particular coverage for each year the employee is employed by Mayfield <br />Village but does not request coverage (e.g., if the annual premium for medical coverage and <br />dental coverage is $10,000.00 and $2,000.00, respectively, an employee who waives both <br />hospitalization and dental shall receive $4,000.00 plus $800.00 in lieu of hospitalization and <br />dental coverage for the year in question). The payment authorized herein shall be made in equal <br />quarterly installments. This payment will not be made to any person who is otherwise covered <br />under the Mayf'ield Village policy through a Village employed spouse or other family member. <br />(Effective in 1994). If an employee is eligible under the terms of this paragraph but has worked <br />less than one full year, the benefit described herein will be prorated on a monthly basis beginning <br />the first month in which the employee would have received the benefit under the plan provided <br />by the Village. <br />(d.) The Village will establish a"Health Care Task Force" comprised of appropriate <br />representatives of the Village administration and Village employees, including representatives of <br />the F.O.P., the Fire Union and representatives from each department. The Task Force will be <br />expected to study the problems of health care and health care cost containment and then <br />formulate recommendations which will be considered by the Village. <br />(e.) Commencing January 1, 2009, every employee covered by family coverage, employee <br />and spouse coverage or employee and children coverage will contribute Fifty and 00/100 Dollars <br />($50.00) per month to the Village for participation in the hospitalization plan. Commencing <br />January 1, 2009, any employee covered by single coverage will contribute Twenty-Five and <br />00/100 Dollars ($25.00) per month to the Village for participation in the hospitalization plan. <br />(f.) The Village, for the term of this agreement, will continue to provide prescription drug <br />coverage with an eight dollar ($8.00) deductible for Generic, a fifteen dollar ($15.00) deductible <br />for Name Brand and a twenty-five dollar ($25.00) deductible for Formulary.
