C'ollective Bargaining Agreement between <br />Mayfield Village and the I.A.F.F. Local2619 <br />ARTICLE 29 <br />gIOSPITALIZATION AND MEDICAL COVERAGE <br />Section 29.1. During the term of this Agreement, the Village may, where economically feasible <br />maintain substantially similar levels of inedical and dental service benefits including prescription <br />riders and life insurance. <br />Section 29.2. It is understood and agreed that the Village will establish a"Health Care Task <br />Force" comprised of appropriate representatives of the Village administration and Village <br />employees, including representatives of the I.A.F.F. The Task Force will be expected to study <br />the problems of health care and health care cost containment and then formulate <br />recommendations which will be considered by the Village. <br />Section 29.3. It is understood and agreed that members of the collective bargaining unit will not <br />make contributions toward the cost of the health care plan for the years 2006, 2007 or 2008. <br />Effective January 1, 2009, employee contributions shall be Fifty Dollars ($50.00) per month per <br />member for family coverage and Twenty-Five Dollars ($25.00) per month per member for single <br />coverage, which shall be deducted from each employee's pay through automatic payroll <br />deduction. <br />The Village, for the term of this agreement, will continue to provide prescription drug coverage <br />with an eight dollar ($8.00) deductible for Generic, a fifteen dollar ($15.00) deductible for Name <br />Brand and a twenty-five dollar ($25.00) deductible for Formulary. <br />Section 29.3. Any permanent full-time employee who is otherwise eligible to be covered under a <br />contract of hospitalization and medical coverage but voluntarily withdraws from the above- <br />referenced insurance coverage shall be paid the amount equivalent to 40% of the premium paid by <br />the Village for the particular coverage for each year the employee is employed by Mayfield Village <br />but does not request coverage (e.g., if the aiuiual premium for medical coverage and dental <br />coverage is $10,000.00 and $2,000.00, respectively, an employee who waives both hospitalization <br />and dental shall receive $4,000.00 plus $800.00 in lieu of hospitalization and dental coverage for <br />the year in question). The payment authorized herein shall be made in equal quarterly installments. <br />This payment will not be made to any person who is otherwise covered under the Mayfield Village <br />policy through a Village employed spouse or other family member. If an employee is eligible <br />under the terms of this paragraph but has worked less than one full year, the benefit described <br />herein will be prorated on a monthly basis beginning the first month in which the employee would <br />have received the benefit under the plan provided by the Village. <br />Page 30 of 38
