- ORDINANCE NO. 91-05 INTRODUCED BY: Mayor Carmen and Council as a Whole AN ORDINANCE AMENDING SECTION 739.03 AND 739.06(b) OF THE CODIFIED ORDINANCES OF MAYFIELD VILLAGE, OHIO, ESTABLISHING A FEE FOR CERTIFICATE OF REGISTRATION AND HOUR3 OF SOLICITATION AT PRIVATE RESIDENCES WITHIN THE MUNICIPALITY, AND DECLARING AN EMERGENCY BE IT ORDAINED BY THE COUNCIL OF MAYFIELD VILLAGE, OHIO, THAT: SECTION 1. Section 739.03 of the Codified Ordinances of Mayfield Village, Ohio, is hereby amended by establishing a fee for a Registration Certificate for purposes of solicitation at private residences within the Municipality. So that the amended Section 739.03 will now read as follows: (Deleted material appears like th'rs and new material appears like THIS) Chapter 739 . Solicitations at Private Residences ? * * * 7391.03 APPLICATION FOR CERTIFICATE; FEES Each applicant for a certificate of registration shall, not less than five nor more than fifteen days prior to the commencement of solicitation furnish the Mayor with the following information on forms provided by the Municipality. (a) Name, age and physical description of applicant. (b) Complete permanent and local address of applicant. (c) Name and address of the person, firm, corporation or association for whom the association for whom the applicant has solicited during the past three years. (d) A description of the nature of a business and the goods, services or wares to be sold otherwise sufficient to identify the subject matter of the soliciting in which the applicant will engage. (e) The names of all other municipalities in which the applicant has conducted soliciting activities during the past six months. (f) Whether the applicant has complied with the requirements of Ohio R.C. Chapter 1716 pertaining to charitable solicitations if applicable.
