EXHIBIT A <br />EXHIBIT A <br />153.11 FAMILY AND MEDICAL LEAVE. <br />(a) Definitions: <br />(1) "Son or Daughter" includes any biological, adopted, or foster child, <br />step- child, legal ward, or child of a person standing in loco parentis who is under <br />18 years of agei or 18 years of age and older and incapable of self care because of i-E <br />r°°d ^f ear° d„° +o a mental or physical disability. <br />(2) "Parent" is either a biological parent or as a person standing in loco <br />parentis. <br />, , ,.<,, , ; <br />(3) "Serious health condition" is a*y i"„°ss°°? <br />,. ++ i; od ,. ,a„i<, ao a+oa hoal+h caro pre.,;alil . an illness, injm, <br />impairment or physical or mental condition that involves either an overni h?y in <br />a medical care facility, or continuing treatment bv a health care provider for a <br />condition that either prevents the employee from performing the functions of the <br />emplovee's job or prevents the qualified familv member from participating in <br />school or other dailv activities. Subject to certain conditions, the continuing <br />treatment requirement mav be met by a period of incapacitv of more than three (3) <br />consecutive calendar days combined with at least two visits to a health care provider <br />or one visit and a regimen of continuing treatment, or incapacity due to pre nancy, <br />or incapacity due to a chronic condition. Other conditions mav meet the definition <br />of continuing treatment. <br />41 "Serious iniurv or illness" is in the case of a member of the Armed <br />Forces, including a member of the National Guard or Reserves, means an injurv or <br />illness incurred by the member in the line of duty on active duty in the Armed <br />Forces that mav render the member medicallv unfit to perform the duties of the <br />member's office, grade, rank, or rating. <br />(b) This The full Village policy, is available from Village administration, <br />describes the family and medical leave benefits available to employees of the <br />Mayfield Village, and shall be }s applicable to all full-time Village employees <br />including those covered by collective bargaining agreements, excluding <br />probationary, part-time, seasonal, and temporary employees.
