I. SCOPE OF SERVICE <br />MAYFIELD VILLAGE will provide all services and assure adherence to the <br />requirements as set forth in Attachments I during the period from June 1, 2009 through <br />and including March 1, 2010, or as adjusted by approved formal request for a project <br />period change, project modification or project extension. Any chaiiges or deviations <br />fi•oxn the attachment must be set forth in writing and approved by the Cuyahoga <br />County prior to implementation. <br />A. Purpose <br />1. The Cuyahoga/LakelMARCSII 7.X next generation system will provide the <br />capability for emergency personnel and first responders to communicate with each <br />other on a daily basis or during an emergency. The system will have a subscriber base <br />consisting of state, local and federal government agencies. Cuyahoga County and <br />MAYFIELD VILLAGE have agreed to utilize Public Safety Interoperable <br />Communications Program funds to achieve this system. <br />2. In order to accommodate Cuyahoga County's desire to participate in this <br />multi-county and state consoilium additional infrastructure needs to be added to the <br />Mayfteld Village Prime Site. Cuyahoga County agrees that MAYFIELD VILLAGE <br />will install the additional Prime Site infrastructure at Cuyahoga County's sole <br />expense, which will enable Cuyahoga County to utiiize the system. <br />B. Responsibilities <br />1. MAYFIELD VILLAGE will purchase and install ihe Exterior Ground Ring <br />and upgraded electrical work to suppoit the 1'rime Site <br />2. MAYFIELD VILLAGE shall submit to Cuyahoga County descriptions of <br />the items purchased for the Prime Site by MAYFIELD VILLAGE, copies of proof of <br />payment and copies of invoices signed by MAYFIELD VILLAGE. 3. The cost of the Prime Site upgrade shall not exceed 4ne Hundred Ten <br />Thousand Doliars and Zero Cents ($110,000.00). <br />C. Required Dooumentation far Reitnbursement Negatiates <br />1. For each item or group of same items purchased by MAYFIELD <br />VILLAGE, copies ofproof of payment and copies of invoices signed by MAYFIELD <br />VILLAGE indicating approval and acceptance of goods shall be submitted to <br />Guyahoga Caunty as required along with the Standard Mon#hly Reporting Form <br />2 af7
