Ordinance 2000-I4 <br />I. Bereavement Leave <br />In the event of death in the employee's immediate family, he shall be entitled to three (3) days <br />off, or in the case of a member of the Fire Department under the rank of Chief, one (1) twenty- <br />four (24) hour shift. An additional two (2) days or for fire, one (1) additional tour, could be <br />granted for out of state funerals or on the approval from the respective Department Head or <br />Chief and the Mayor. The "immediate family" shall include the spouse, children, parents, <br />parents-in-law, brothers, sisters, grandparents, brothers-in-law and sisters-in-law. Should an <br />exception to the aforementioned apply, such exception must be authorized by the Mayor. An <br />employee's use of this leave while on vacation, holiday, or sick leave, shall result in the <br />employee not being charged for such vacation, holiday or sick leave. <br />J. Hospitalization and Medical Coverage <br />1. The Municipality shall pay the premiums for all permanent full-time employees insured <br />under a contract of hospitalization and medical coverage selected by Mayfield Village. <br />2. Permanent full-time employees of the Municipality not presently insured and new permanent <br />full-time employees of the Municipality may join the plan at such intervals as may be <br />authorized by the Municipality's medical insurance carrier, subject to the approval of the <br />Mayor and Finance Director. <br />3. Any permanent full-time employee who is' otherwise eligible to be covered under a contract <br />of hospitalization and medical coverage but voluntarily withdraws from the above- <br />referenced insurance coverage shall be paid the amount of One Thousand Dollars <br />($1,000.00) for each year the employee is employed by Mayfield Village but does not <br />request coverage. For the time period covered by this ordinance, dental coverage will not be <br />considered as part of the hospitalization or medical coverage described above. The <br />payment authorized herein shall be made in equal quarterly installments. This payment will <br />not be made to any person who is otherwise covered under the Mayfield Village policy <br />through a Village employed spouse or other family member. (Effective in 1994). If an <br />employee is eligible under the terms of this paragraph but has worked less than one full <br />year, the benefit described herein will be prorated on a monthly basis beginning the first <br />month in which the employee would have received the benefit under the plan provided by <br />the Village. <br />2000-2002 Wage Ordinance: 2000-14 <br />Page 20 of 25 <br />
