WORK ORDER N0.4 <br />NORTHWEST QUADRANT PROJECT -S.O.M. CENTER/WHITE ROAD WIDENING <br />ENVIRONMENTAL CLEARANCE DOCUMENTS <br />URS GREINER /HAYFIELD VILLAGE <br />AGREEMENT DATED FEBRUARY 3,1998 <br />This Work Order Task is being performed as an extension of the Professional Engineering Services <br />Agreement between Mayfield Village and URS Greiner (URSG) dated February 3, 1998. All terms and <br />conditions stated therein aze incorporated into this Work Order by reference. <br />UNDERSTANDING OF THE PROJECT <br />The project consists of widening White Road between S.O.M. Center Road and I-271 to three lanes, widening <br />of S.O.M. Center Road from White Road to Highland Road, to five lanes. Signalization of four intersections <br />along S.O.M. Center Road and White Road aze included. Most of the project will be located within existing <br />rights-of--way although additional strip right-of--way takes may be required to meet applicable design <br />standazds. <br />In developing the Scope of Services and cost proposal, certain assumptions have been made regarding tasks <br />to be performed by URSG and the overall level of effort. Specific assumptions. upon which the proposed <br />Scope of Services is based consist of the following: <br />The following guidance documents were considered in preparing the Scope of Services. <br />Federal Highway Administration, Guidance for Preparing and Processing Environmental and <br />Section 4(f) documents, dated October 30, 1987. <br />The following deliverables were assumed to be required as part of the Basic or Optional Services. The <br />maximum number of reports/analyses assumed for estimating the level of effort is indicated in parentheses: <br />- Hazazdous Waste Screening Report (1 document)(basic) <br />- Phase I Environmental Site Assessment (3 sites)(optional) <br />- Ecological Survey Report and Ecological Report Addendum (2 documents)(basic) <br />- Phase I (formerly Phase I & In Cultural Resources Survey (1 report)(basic) <br />- Phase I Environmental Site Assessment (1 document)(optional) <br />- Noise Analysis (1 document)(optional) <br />- Environmental Assessment of Feasible Alternatives (1 document)(basic) <br />- Phase II Cultural Resources Report (1 site)(optional) <br />- Draft Environmental Assessment (1 document)(basic) <br />- Final Environmental Assessment (1 document)(basic) <br />Based on URSG's understanding of the project azea, no Section 4(f)'s and 106/Section 4(f) evaluations aze <br />required. <br />The proposed action will not require any relocations, therefore a conceptual relocation survey will not be <br />required. An optional RAP task has been included in the event this assumption turns out not to be true. <br />Specific services aze excluded from the Scope of Services described herein; Air Quality, Major Investment <br />Study (MIS), Flood Hazazd/EncrBachment Study.
