base mapping provided by Mayfield Village. The results of coordination with the State Historic <br />Preservation Officer, Soil Conservation Service, U.S. Environmental Protection Agency, and other <br />agencies shall be incorporated into the assessment. URSG shall complete the farmland coordination <br />as part of the preparation of the EAFA. Finally, the results of the public involvement meeting shall <br />be incorporated into the EAFA. A total of five (5) copies of the EAFA shall be submitted for review <br />and comment. <br />STAGE III -EVALUATION FOR SIGNIFICANCE <br />Most of the Stage III work shall consist of revising the EAFA in response to comments provided by ODOT. <br />Each of the tasks to be performed by URSG is briefly described below. <br />A. Cultural Resources -Prehistoric Resources: The subconsultant shall complete a Phase III <br />investigation for one prehistoric site in accordance with ODOT and State Historic Preservation Office <br />Procedures. Five (5) copies of the subject report shall be submitted to URSG for forwarding to <br />Mayfield Village. The results of the investigation and coordination with the State Historic <br />Preservation Officer shall be incorporated into the Environmental Assessment. <br />B. Ecological -Wetland Delineation: URSG shall delineate all jurisdictional wetlands potentially <br />impacted by the total project under another task . For estimate purposes, upwazds of one (1) acre of <br />wetlands shall be delineated. A separate wetland delineation report will be prepared and five (5) <br />copies submitted to Mayfield Village for forwazding to ODOT. Upon acceptance, the necessary <br />additional copies of the report shall be provided for completing coordination. <br />C. EAFA Revision to Draft Environmental Assessment: URSG shall revise the EAFA to advance <br />the project to the public hearing (if required). For estimating the level of effort, no major revisions <br />will be required by ODOT or FHWA although supplemental minor editorial revisions and additional <br />information may be required . to be added to the EA regazding the projects impacts on cultural <br />resources. Once completed, a total of eighteen copies (18) copies of the draft environmental <br />assessment shall be provided to Mayfield Village. Sixteen (16) of the copies shall be forwarded to <br />the Ohio Department of Transportation. <br />D. Public Hearin: If a public hearing is held, URSG shall prepare displays and informational <br />handouts. URSG shall also attend a meeting in advance of the public hearing to fmalize hearing <br />logistics and assist Mayfield Village in setting up the meeting. <br />E. Final Environmental Assessment: URSG will revise the draft EA after receipt of any agency and <br />citizen comments received as a result of the public notice and/or the holding of the public hearing, <br />if required. For estimate purposes, it is assumed that the document shall be fmalized as an <br />environmental assessment and only minor revisions to the environmental assessment will be required. <br />Responses to comments will be developed by URSG. A maximum of twenty (20) copies of the final <br />environmental assessment shall be submitted to Mayfield .Village for forwarding to the Ohio <br />Department of Transportation for final review and approval. URSG shall also prepaze for Mayfield <br />Village, the draft finding of no significant impact (FONSI) to accompany the submission. <br />F. Project Meetings: This activity will consist of all project related meetings with the Mayfield Village <br />and ODOT. Excluded aze public input meetings and meetings with outside reviewing agencies. A <br />maximum of six (6) meetings are assumed required.
