Task (5) -Interviews: Persons determined to have knowledge of spills, releases, or <br />chemical management.. at a given nonresidential property within the right-of--way will be <br />interviewed. If deemed appropriate, adjacent facilities/residents will also be interviewed for <br />the same purpose. Residential dwellings will not be solicited for interviews unless <br />information becomes available that indicates involvement with hazardous materials or <br />becomes apparent from the site walkover. <br />Task (6) -Site Visits: A detailed site walkover will be conducted by URSG employee. The <br />individuals will have OSHA 40-hour training in accordance with 29 CFR 1910.120 in <br />addition to actual site assessment experience. The site walkover will cover only suspect <br />properties recommended for Phase I investigation. Notes of the walkover will be recorded <br />on field maps carried by each, individual. <br />The field group will use an HNU-101 volatile organic compound detector and a TIF 8800 <br />explosive gas detector for the walkover. If encountered, areas such as spills or suspect <br />contamination sites may be hand augured to a maximum depth of 1.5 ft and field screened <br />with the instruments. Readings and locations will be recorded on the maps. Samples will <br />not be taken for laboratory analysis as part of the Phase I program. In addition, photographs <br />will be taken in the field to document conditions. <br />Task (7) - Phase I Report: At the conclusion of tasks 1 - 6, provide a comprehensive <br />written report documenting the findings and conclusions of the Phase I site investigation. <br />The report will address the study methodology, past and present land uses and their <br />relationship to possible contaminants within the right-of--way, and a comprehensive listing <br />of potentially contaminated sites that may require Phase II investigations. As noted under <br />Assumptions, Phase II ESA is not considered required for this project therefore, it is assumed <br />it will not be necessary to prepare a Phase II SAP. <br />D. Stage II. Feasible Alternatives Engineering; Three optional tasks are proposed under this Scope <br />item: 1) Interim Congestion Management System evaluation, 6) Noise Analysis, and 8) Conceptual <br />Relocation Assistance Program. <br />(6) Noise Analysis: In the event that ODOT decides not to perform a noise analysis on <br />behalf of the Mayfield Village, URSG shall conduct a noise analysis pursuant to ODOT and <br />FHWA guidelines for conducting noise analyses. It is anticipated that where specified <br />exceedances of desirable noise levels are identified, that mitigation measures will be <br />preliminarily investigation including noise barrier walls. A maximum of 18 locations shall <br />be evaluated/modeled for existing and future build and no-build noise levels. Noise impacts <br />on platted but not constructed subdivisions will also be investigated. Four (4) copies of the <br />draft report shall be submitted to the Mayfield Village for forwarding to ODOT for review. <br />In response to revisions provided by ODOT, a final noise analysis report shall be prepared <br />and resubmitted for final approval. A total of four (4) copies of the final noise analysis shall <br />be provided to Mayfield Village. <br />(8) Conceptual RAP: In the event that a RAP is required and ODOT decides not to <br />perform a Conceptual RAP, URSG shall prepare the Conceptual RAP. The RAP shall be <br />submitted to Mayfield Village for submittal to ODOT for approval. <br />E. Environmental Assessment of Feasible Alternatives .(EAFAI: An environmental assessment of <br />feasible alternatives will be prepared which incorporates the findings of previously approved reports <br />and incorporates additional information developed in Stage I and [I.
