SECTION IV SCHEDULE OF BENEFITS <br />4.01. Annual benefits provided by the plan <br />Each Participant shall be entitled to reimbursement for Eligible Medical or Prescription Expenses <br />incurred during the Plan Year as outlined below. <br />Eligible Medical or Prescription Expenses are those benefits which are eligible for reimbursement <br />under the employer sponsored fully insured health program but are not reimbursed because they <br />are applied to the participant out-of-pocket expense of the fully insured plan. Reimbursement is <br />subject to the maximum benefit available in each employee's Health Reimbursement Account <br />(HRA). Employer shall fund the Health Reimbursement Accounts as identified in this section. <br />MAXIMUM REIMBURSEMENT" The maximum benefit reimbursement per individual by this plan <br />is listed below: <br />Mayfield Village HRA Plan <br />$2,000 per year for all claims (2x per family or other than single coverage). <br />Eligible medical and prescription expenses only. <br />Funds are forfeited upon termination or end of calendar year. <br />4.02. Cost of coverage <br />The Employer shall bear the entire expense of providing this Plans benefits set out in Section <br />4.01. <br />SECTION V PAYMENT OF BENEFITS <br />5.01. Eligibility for benefits <br />Each Participant in the Plan shall be entitled to a benefit hereunder for all Eligible Medical or <br />Prescription Expenses incurred by the Participant on or after the effective date of his participation, <br />(and after the effective date of the Plan) subject to the limitations contained in Section V, below, <br />provided that the expense is eligible for coverage under the employer sponsored fully insured <br />policy. <br />5.02. Claims for benefits <br />It shall be the Employees responsibility to assure that all claims qualify under section 213(d).
