SECTION IV SCHEDULE OF BENEFITS 4.01. Annual benefits provided by the plan Each Participant shall be entitled to reimbursement for Eligible Medical or Prescription Expenses incurred during the Plan Year as outlined below. Eligible Medical or Prescription Expenses are those benefits which are eligible for reimbursement under the employer sponsored fully insured health program but are not reimbursed because they are applied to the participant out-of-pocket expense of the fully insured plan. Reimbursement is subject to the maximum benefit available in each employee's Health Reimbursement Account (HRA). Employer shall fund the Health Reimbursement Accounts as identified in this section. MAXIMUM REIMBURSEMENT" The maximum benefit reimbursement per individual by this plan is listed below: Mayfield Village HRA Plan $2,000 per year for all claims (2x per family or other than single coverage). Eligible medical and prescription expenses only. Funds are forfeited upon termination or end of calendar year. 4.02. Cost of coverage The Employer shall bear the entire expense of providing this Plans benefits set out in Section 4.01. SECTION V PAYMENT OF BENEFITS 5.01. Eligibility for benefits Each Participant in the Plan shall be entitled to a benefit hereunder for all Eligible Medical or Prescription Expenses incurred by the Participant on or after the effective date of his participation, (and after the effective date of the Plan) subject to the limitations contained in Section V, below, provided that the expense is eligible for coverage under the employer sponsored fully insured policy. 5.02. Claims for benefits It shall be the Employees responsibility to assure that all claims qualify under section 213(d).
