SECTION III ELIGIBILITY <br />3.01. General requirements <br />Each Employee who has enrolled in the qualifying Employer sponsored fully insured benefit plan <br />under which accident and health benefits are provided, is eligible to participate in this Plan as of <br />the Effective Date or any subsequent date which coordinates with the effective date of the <br />primary, fully insured benefit plan. <br />3.02. Reentry after uniformed service duty <br />No reentry eligibility requirements will be imposed on any Employee who returns to active <br />employment within 90 days of completing a period of absence from employment for duty in the <br />Uniformed Services. <br />3.03. Termination of a Participant's coverage <br />Except as provided in Section VI, coverage of a Participant shall terminate automatically on the <br />date that the Participant-- <br />(a) no longer is eligible to participate in the employer sponsored fully insured program, based <br />upon the eligibility requirements set forth therein; <br />(b) has declined participation in the qualifying employer sponsored fully insured program; <br />(c) is not covered because the provisions of the Plan terminated; <br />(d) and/or is no longer an employee and has exhausted his/her Health Reimbursement Account <br />Balance. <br />3.04. Termination of coverage of an Eligible Dependent <br />Except as provided in Section VI, an Eligible Dependent's coverage shall terminate-- <br />(a) on the dates described in Section 3.03, as if the references to "Participant" were to "Eligible <br />Dependent"; <br />(b) for an Eligible Dependent other than the Spouse of a Participant, coverage shall <br />automatically terminate when an individual who had been an Eligible Dependent no longer <br />qualifies as such based upon the eligibility requirements of the employer sponsored fully insured <br />plan. <br />The dates referred to in this Section may be modified by the terms of Section VI CONTINUATION <br />COVERAGE. <br />Mayfield Village HRA Plan 0117.doc <br />
