All employees receiving dental benefits will pay, through payroll deduction, a <br />contribution equal to 8% of the premium for the dental plan. Effective January 1, 2019, <br />employees selecting the Metro Health Plan will pay, through payroll deduction, a contribution <br />equal to 6% of the premium. Employees selecting the 2000/4000 Medical Mutual Plan will pay, <br />through payroll deduction, a contribution equal to 9% of the premium. All employees receiving <br />dental benefits will pay, through payroll deduction, a contribution equal to 9% of the premium <br />for the dental plan. <br />Section 28.4. Any permanent full-time non-probationary employee, who is eligible to be <br />covered under a contract of hospitalization and medical coverage but voluntarily withdraws from <br />coverage, shall be paid Three Hundred Dollars ($300.00) per month in lieu of family coverage <br />and One Hundred Dollars ($100.00) per month in lieu of single coverage. These payments shall <br />be made in equal quarterly installments. These payments will not be made to any person who is <br />otherwise covered under the Employer's policy through a spouse or other family member. No <br />payment can be made under this section until the employee provides proof to the Finance <br />Director that he or she is covered under another health insurance policy. <br />Section 28.5. The Employer will provide and pay the full premium on a $20,000 term life <br />insurance policy for each permanent full-time employee. <br />14IdTICLE 29 <br />I1~.IIIIlI' LE~4 VE <br />Section 29.1. When an employee becomes injured or disabled as a result of an event arising out of <br />and in the course of bona fide police work as determined by the Village, so as to be physically <br />unfit for duty, the employee shall be granted a special leave of absence with pay by the Village <br />beginning with the first working day of such disability. In order to be eligible for the special on <br />the job injury leave, as provided in the article, the employee's disability must be evidenced by a <br />certificate of a physician who examined the employee. Special on the job injury leave shall not be <br />granted to employees who incur injuries of a routine nature or to employees while in the <br />employment of another person or entity. <br />Section 29.2. Special on the job injury leave shall not exceed Ninety (90) consecutive calendar <br />days after the beginning of the leave unless an extension is granted by the Employer. Said leave <br />maybe canceled at an earlier time as provided below: <br />A. When the employee is released by his physician to return to work; <br />B. At such time that the employee is declared capable of performing his normal duties <br />by a physician appointed by the Employer; <br />25 <br />
