Agreement between Mayfield Village and F.O.P. Lodge 57 <br />days shall not be deducted from the employee's sick leave credits. If the employee is regularly <br />scheduled to work all three days, the employee will be compensated for all three days. An <br />additional two (2) days of leave maybe granted for out-of--state deaths upon approval by the Chief <br />of Police and Mayor. <br />Section 27.2. In the event the death occurs during the employee's work day, he shall be granted <br />the remaining portion of the day off with pay. Such time shall not be deducted from any of the <br />employee's sick leave credits. <br />Section 27.3. Immediate "family" shall be defined as only including the employee's spouse, <br />children, parents, parents-in-law, brothers, sisters, grandparents, brothers-in-law and sisters-in- <br />law. <br />Section 27.4. The use of this leave is a privilege which must be specifically requested through the <br />Chief of Police. <br />Section 27.5. Incases where more time off is desired than was granted, the employee shall request <br />in advance, the use of his accrued vacation, sick time, holidays, compensatory time and/or <br />sick/personal time as credits for such additional time. <br />14R7'ICLE 28 <br />INS'UI~41~CE,S' <br />Section 25.1. Effective January 1, 2017, the Village will offer to all eligible employees, a high <br />deductible medical plan, a dental plan and prescription drug benefits that include afully-funded <br />Health Reimbursement Arrangement (HRA) limited to medical and prescription drug expenses <br />and a Medical Expense Reimbursement Plan (MERP) option funded at 50% of the 2000/4000 <br />deductible amount. Effective January 1, 2018, the Village will offer to all eligible employees, a <br />high deductible medical plan, a dental plan and prescription drug benefits that include afully- <br />funded Health Reimbursement Arrangement (HRA) limited to medical and prescription drug <br />expenses. <br />Section 28.2. It is understood and agreed that the Village will continue to utilize a "Health Care <br />Task Force" comprised of appropriate representatives of the Village administration and Village <br />employees, including representatives of the F.O.P. The Task Force will be expected to study the <br />problems of health care and health care cost containment and then formulate recommendations <br />which will be considered by the Village. <br />Section 25.3. Effective January 1, 2017, employees selecting the Metro Health Plan will pay, <br />through payroll deduction, a contribution equal to 5% of the premium.
