_ ~' HAYFIELD VILLAGE ' INVITATION TO BID FOR RESIDENTIAL SOLID WASTE COLLECTION, DISPOSAL AND RECYCLING SERVICES FIRST ADDENDUM March 24, 2017 This addendum summarizes questions presented at the pre-bid meeting held on March 24, 2017 and amends, as follows, the Invitation to Bid for Residential Solid Waste Collection, Disposal and Recycling Services issued by Mayfield Village on March 16, 2017. ITB Amendments 1. Replace Section 1. F. with the following: The term of the Contract will be (5) years. The commencement date of the Contract will be June 15, 2017 and will terminate on June 14, 2022. 2. Remove Section ll. A., paragraph 11: The Contractor shall collect and recycle scrap tires. 3. Replace Section 11. D., Table 2: Current Container Services with the revised table which is attached. 4. Replace Section 1V. C., with the following: ~~~ The Contractor after receiving a Notice of Award, will furnish a Performance Bond in the amount of $500,000 executed by a duly authorized surety, acceptable to the Village in all respects, or such other security acceptable to the Village. The Performance Bond will be issued annually for each contract year during the term of the contract. The entire cost of the bond(s) will be paid for by the Contractor. 5. Replace Bid Form 3 Price Sheet: Automated Solid Waste, Disposal and Recycling Collection with the l revised bid form which is attached. + 6. Replace Bid Form 4 Price Sheet: Container Services with the revised bid form which is attached. 7. In the Form of Contract, Replace Article ll. 1. Term with the following: This Agreement shall be effective upon the date last signed below. The Commencement date for Residential Solid Waste Collection and Recycling Services is June 15, 2017 and the initial term of this Agreement shall terminate on June 14, 2022. 8. In the Form of Contract, Remove Article ll. 2. Renewal Terms. 9. In the Form of Contract, Remove Article lll. 1., paragraph 6: The Contractor shall collect and recycle scrap tires. 10. In the Form of Contract, Replace Article IV, Table 1: Price for Weekly Waste and Recycling Collection and Table 2: Disposal Cost per Ton with the revised tables which ore attached.
