Collective Bargaining Agreement between <br />Mayfield Village and the LA. F. F. Local 2619 <br />Section 12.6. Records of disciplinary action shall cease to have force and effect or be considered <br />(W in future discipline matters under the following timeframes: <br />Oral and written reprimands <br />12 months <br />Suspensions of less than 3 tours <br />18 months <br />Suspensions of 3 tours or more <br />30 months <br />ARTICLE 13 <br />PERSONNEL FILES <br />Section 13.1. It is recognized by the parties that the Employer may establish regulations for the <br />custody, use and preservation of the records, papers, books, documents and property pertaining to <br />the Employer or its employees. Employees shall have access to their own individual personnel files <br />for review of documents contained in said personnel file. An employee shall not have access to any <br />other employee's personnel file. Employees shall have access to their individual personnel files for <br />review in the following manner: <br />1. Requests for review must be in writing. Time and date for review is subject to <br />the availability of the employee's personnel file. Such review shall not last <br />longer than thirty (30) minutes. <br />2. All reviews shall be conducted during normal business hours. <br />3. Employees have the right toPY an a rocure co of and all articles in his <br />P Y <br />personnel file, copies of which shall be provided by the Employer. <br />Employee personnel files shall include, but may not be limited to, individual employment data, <br />payroll information, work time schedules, records of additions or deductions paid, application <br />forms, records pertaining to hiring, promotion, demotion, transfer, layoff and termination. <br />Section 13.2. Nothing herein shall prevent the dissemination of impersonal statistical <br />information. <br />Section 13.3. Should an employee dispute any of the contents of his or her personnel file, he or <br />she may attach a written rebuttal to the disputed item for inclusion into the file within thirty (30) <br />days of the date that the employee learns that the item was placed in his or her personnel file. <br />ARTICLE 14 <br />GRIEVANCE PROCUDURE <br />Section 14.1. It is mutually understood that the prompt presentation, adjustment, and/or <br />answering of grievances is desirable in the interest of sound relations between the Employer and <br />the Union. The procedures specified in this Article are intended to provide a system for a fair, <br />expeditious, and orderly adjustment of grievances of the employees who are Union members in the <br />Fire Department. <br />
