Collective Bargaining Agreement between <br />Mayfield Village and the I.A. F. F. Local 2619 <br />of its political subdivisions, providing a letter of verification from the prior public employer is <br />submitted within the first (I") year of employment with the Village, is entitled to have such <br />prior service counted as years of employment for the purpose of computing vacation time <br />herein and shall be credited as such on January 1 st after initial year of employment with the <br />Village. <br />ARTICLE 29 <br />HOSPITALIZATION AND MEDICAL COVERAGE <br />Section 29.1. All permanent full-time employees shall be eligible to participate in a group health <br />care plan established by the Employer, which shall include medical, dental, vision, and <br />prescription drug coverage. Such group plan may be provided through a self-insured plan or an <br />outside provider, or a combination thereof. In each year of the contract, the Employer shall offer <br />a base plan and may offer alternate plans. <br />Section 29.2. Health Care Plan. In 2020, the base plan being offered by the Employer is the <br />Buckeye Ohio Risk Management Association Benefits Pool, Inc. (BORMA) Aetna Choice POS <br />II — Standard Plan 2. The 2020 base plan includes an in -network out-of-pocket limit of $600 for <br />single coverage and $1,200 for employee plus dependent coverage (hereinafter the "2020 out-of- <br />pocket limit"). The 2020 out-of-pocket limit will be fully -funded by the Employer with a Health <br />Reimbursement Arrangement (HRA). To help offset the loss of the HRA, provided there is no change in the <br />2020 out-of-pocket limit, any costs above the Total Premium amounts set forth above shall be <br />paid ninety-one percent (91%) by the Employer and nine percent (9%) by the employee and the <br />Employer will reduce the Employee Premium Contribution by 50% on the adjusted total amount. <br />Section 29.3. Dental and Vision Plans. In 2020, all employees receiving dental and vision <br />benefits will pay, through payroll deduction, a premium contribution equal to nine percent (9%). <br />In 2021 and 2022, provided there is no change in the 2020 out-of-pocket limit, and to further <br />offset the loss of the HRA, the employee contribution toward dental and vision premiums will be <br />reduced to zero percent (0%). <br />Section 29.4. Health Care Task Force. The Employer will continue to utilize a Health Care <br />Task Force comprised of appropriate representatives of the Village administration and Village <br />W <br />
