Section 28.4. Health Care Task Force. The Employer will continue to utilize a Health Care Task <br />Force comprised of appropriate representatives of the Village administration and Village <br />employees, including representatives of FOP Lodge 57 and IAFF Local 2619. The Health Care <br />Task Force will be expected to study the issues of health care and health care cost containment and <br />formulate recommendations which will be considered by the Employer. <br />Section 28.5. Any permanent full-time employee, who is otherwise eligible to be covered under <br />the group health care plan and who voluntarily withdraws from the above -referenced insurance <br />coverage, shall be paid Three Hundred Dollars ($300.00) per month in lieu of employee plus <br />dependent coverage and One Hundred Dollars ($100.00) per month in lieu of single coverage. <br />These payments shall be made in equal quarterly installments. These payments will not be made <br />to any person who is otherwise covered under the Employer's policy through a spouse or other <br />family member. No payment can be made under this section until the employee provides proof to <br />the Finance Director that he or she is covered under another health insurance policy. <br />Section 28.6. The Employer will provide and pay the full premium on a $20,000 term life <br />insurance policy for each permanent full-time employee. Commencing April 1, 2020, the <br />Employer will increase the value of each policy and provide and pay the full premium on a $50,000 <br />term life insurance policy for each permanent full-time employee. <br />ARTICLE 29 <br />INJURY LEAVE <br />Section 29.1. When an employee becomes injured or disabled as a result of an event arising out <br />of and in the course of bona fide police work as determined by the Village, so as to be physically <br />unfit for duty, the employee shall be granted injury leave with pay by the Village beginning with <br />the first working day of such disability. In order to be eligible for injury leave, as provided in this <br />Article, the employee must file a claim with Workers' Compensation and the employee's <br />disability must be evidenced by a certificate from a physician who examined the employee. Paid <br />injury leave shall not be granted to employees who incur injuries of a routine nature or to <br />employees while in the employment of another person or entity. <br />Section 29.2. Injury leave shall not exceed Ninety (90) consecutive calendar days after the <br />beginning of the leave unless an extension is granted by the Employer. Said leave may be canceled <br />at an earlier time as provided below: <br />A. When the employee is released by his physician to return to work; <br />B. At such time that the employee is declared capable of performing his normal duties <br />by a physician appointed by the Employer; <br />C. If, prior to release for normal duties, it is determined by a physician that the <br />employee is capable of performing limited work assignments, the employee shall <br />23 <br />
