HEALTH
SCIENCES
CENTER
2001
Optometry Clinics
Mr. Thomay
Chairman, North Olmsted Civil Service Commission
5200 Dover Center Road
North Olmsted, OH 44070
Dear Mr. Thomay:
338 West Tenth Avenue
Columbus, OH 43210-1240
Phone: (614) 292-2020
FAX: (614)688-4960
I am writing this letter at the request of Mr. David Stein, who was examined at The Ohio
State University College of Optometry Diagnosis and Management of Ocular Disease
Service on June 15, 2001. The reason for this visit was to determine if Mr. Stein meets
the color vision criteria specified by the Commission for employment as a firefighter in
North Olmsted, OH.
The examination revealed vision of 20/20 OD/OS without correction. Color vision
testing revealed red -green defect by Pseudoisochromatic Plate series. The D-15 test
revealed several major crossing errors that aligned with the Deutan axis. The Farnworth
Lantern revealed 7 errors on one run (where one error/per run on two runs indicate a
pass). Finally, the Nagel Anomaloscope revealed brightness and color matching that
corresponded with a moderate deutanomalous defect (anomalous trichromacy type).
My assessment is that Mr. Stein possesses a moderate deutanomalous defect, which
meets the specified criteria and should not interfere with his job requirements.
I recommend that Mr. Stein be allowed to serve on the North Olmsted firefighters
department based on results of testing acquired at this visit.
Sincerely,
ehL_/_q'
Cynthia Green, O.D.
Asst.
