d. The eligible person fails to respond to a notice from the Civil Service Commission within a <br />deadline provided. <br />e. The eligible person declines an appointment without reasons satisfactory to the Civil <br />Service Commission. <br />f The eligible person fails to inform the Commission by written notice of any change of <br />his/her address. <br />g. The eligible person cannot be located by the postal authorities. <br />h. The eligible person fails to be appointed after being certified five (5) times. <br />i. The eligible person does not pass the background investigation. <br />j. The eligible person for position of firefighter or police officer fails to pass physical agility <br />test or fails to maintain valid certification. <br />k. The eligible person fails to meet age requirements for Entry -Level police and fire <br />department positions. <br />L. The eligible person fails any other requirement as specified by the Commission or the <br />Appointing Authority. <br />RULE VIII <br />APPOINTMENTS <br />CERTIFIED LIST OF ELIGIBILITY <br />Section 1: Opening in a Classified Position: <br />Whenever an opening occurs in a classified position, the Appointing Authority shall notify <br />the Civil Service Commission of the opening and shall request the appropriate number of <br />names from the existing eligible list for the position. Following notification by the <br />Appointing Authority of an opening in a classified position, the Commission shall, except <br />as otherwise provided for within Ohio Revised Code Sections 124.44 and 124.45, certify <br />to the Appointing Authority the names, addresses, and rankings of the five (5) highest <br />applicants on the eligible list for the position. If a valid list does not exist the Commission <br />shall arrange for a test to be given. <br />Section 2: Appointments: <br />All original and promotional appointments to positions within the classified service shall <br />be made from names certified to the Appointing Authority by the Civil Service <br />Commission. <br />a. Upon receipt of such certified names, addresses and rankings from the Commission, the <br />Appointing Authority shall fill open position by appointing one (1) of the five (5) persons <br />certified to him, and shall immediately notify the Commission in writing of the name of <br />15 05-24-2004 Update <br />
