ATTACHMENT A - MOBILE MAINTENANCE SERVICES <br />A) Maintenance services deals with answering questions and helping <br />resolve issues with the system. <br />B) On -Site or via remote connection. With remote connection, <br />customers receive immediate and updated technical support <br />and answers to their questions. Customers open the connection <br />so ICES consultants can directly access their system and evaluate <br />the problem on -line. Once the consulting session is finished the <br />customer closes the connection. <br />C) Compulhensive Business Solutions will, as a part of the Maintenance Agreement, pick <br />malfunctioning hardware up from the client site and contact Compaq for assistance and <br />return merchandise if required. CBS, Inc. agrees to pay all shipping and handling costs <br />associated with a return of equipment. We are not directly nor indirectly responsible for <br />damaged or malfunctioning equipment, the manufacturer is ultimately responsible for <br />their equipment. Maintenance Services relating to hardware will only be available if the <br />manufacturer warranty is in place, CBS will provide manufacturer warranty cards and <br />date of purchase. <br />D) Licensor enhancements and upgrades, including Help <br />documentation as specified in this agreement. <br />Third -Party product enhancements as required. <br />E) Annual Maintenance Fee — ICES <br />15% of ICES Software $1,339.00 <br />F) Annual Maintenance Fee — ICES Mobile <br />10% of ICES Mobile Software <br />($10,493.00 (.10) = $1,049.00) $1.049.00 <br />$2,388.00 <br />G) Maintenance Service Term Start Date <br />Terminate Date <br />ANNUAL MAINTENANCE FEE SCHEDULE <br />ANNUAL SUPPORT <br />ICES Mobile (price per unit / user) <br />1 Users <br />15% <br />2 -4 Users <br />12.5% <br />5 -9 Users <br />10% <br />10+ Users <br />8% <br />
