? <br />14 <br />B. Documentation Required. The following documentation should be mailed to <br />HUD at the time of reporting the closure to HUD: <br />1. A chronology of agency actions prior co the withdrawal request; and <br />2. A copy of the signed and dated withdrawal rtquest. <br />V. ADMINISTRATIVE CLOSURES <br />A. Witlldrawals Without Resolution <br />1. Requir•ements for Acceptance. The withdrawal request must: <br />a. Be in writing; <br />b. Be signed and dated by the complainant or an authorized . <br />representati ve; <br />c. Identify the respondent(s); <br />d. Contain the HLTD case number and the State or local agency case <br />number; <br />e. Indicate the reason(s) why complainant wishes to withdraw the charge <br />(see also VI.3 of these Criteria); <br />f. Contain a specific reference to the withdrawal of the HLTD complaint <br />as well as the agency's complaint; and <br />g. Indicate the complainant's awareness that the withdrawa] will result in <br />the teimination of proceedings involving the matter; and <br />h. Contain a specific statement that there was no coercion or fear of <br />retaliation from any person (not jusc the respondent). <br />2. Documentation Required (To be mailed to HUD at time of reporting the <br />closure to HUD): <br />a. A chronology of agency actions prior to withdrawal request; and <br />b.
