<br /> <br />The SERVICE DEPARTiVIENT agrees to operate the equipment per our <br />recommendations and to permit only LK HEATfNG & COOLING, INC. technicians <br />to work on the equipment. <br />EXCLUDED FR4M THIS AGREEMENT ARE THE FQLLOWlNG: <br />Any repairs required because of negligence, misuse of equipment or because of <br />fire, flood, Acts of God, shortages of electrical, gas or water supply, sabotage or <br />damage caused by freezing. <br />This Agreement becomes effective when signed and paid, and continues for one <br />(1) year, with a one year renewal at City's option. <br />The aforementioned service will be pravided for the sum of: <br />FIVE HUNDRED NINETY DOLLARS <br />($590.00) <br />Accepted: ?--, ' • ??.
