Vision - Option Pian #2 (vision pian simitaz to that offered thmugh Union Eye Care) <br />3. Dental and Vision (Option Plan #1 or #2) <br />Full-time Employees parricipating and subscribing to the optional health benefit listed above <br />within this Section shall contribute an equal amount to that which is listed in Section 19.2(A), (B) <br />and (C) of this Article. Employees shall be required to participate within the optional benefit <br />coverage listed within this Section. Tfie Empiayee contribution shall be withheld as a payroll <br />deduction not later than the first pay period of each month. <br />19.5 Expected cost of premium is defined as the cost of all medical, hospitai, prescription drug and <br />retated fees inctuding, but not limited to, administration fees. <br />19.6 The Employer reserves the right to estabiish plans, enroltment periods and regulations. <br />19.7 The Employer shall establish an Insurance Committee of one (1) to two (2) repcesentarive from <br />each of the City's bargaining units, if they choose to be represented and a minimum of one (1) <br />representative of the Employer. The Committee shall meet at least once a month for the purpose <br />of expIoring cost saving measures and/or alterative health plans. The Committee shall make <br />recommendations regarding health care caverage and such recommendations shall be presented to <br />each bazgaining unit as well as to the City Administration. <br />19.8 All changes affecting the plan coverage's capayment, deductibles, providers' enrollment periods, <br />and regulations will be shared with the bargaining unit prior to making such changes. <br />199 It is further agreed that if any bazgaining unit employed by the City of North Olmsted, in <br />negotiations held subsequent to this Agreement and for ihe same time period covered under this <br />Agreement, negotiates to conclusion a iower percentage af Employee contribution to the health <br />Insurance Plan, that lower percentage shall apply to the unit covered under this Agreement. Said <br />lower percentage shall commence under this Agreement at the time that it is agreed upon by <br />anather unit, and shall continue for the remaining years covered under this Agreement. <br />19.10 All Employees shall receive $25,000.00 in group term life insurance, paid for by the Employer. <br />19.11 The city shall cause to be implemented the current Health Care Plan as a qualified Plan under <br />Section 125 of the Intemal Revenue Code. <br />34
