ARTICLE 12 <br />HEALTH INSURANCE <br />12.1 The employer shall provide either individual or family medical insurance coverage <br />at the employee's option for each full-time employee. The employer shall have the option af <br />seeking alternate health insurance providers or plans and will attempt to offer at least two <br />different plans. Basic hospitalization will be consistent with and similar to those plans offered on <br />July l, 1994. Basic hospitalization will be that affered to other employees of the City of North <br />Olmsted. The employer has the right to negotiate co-payment and prescription drug rates with <br />providers to lower the cost to the emplayer as well as the employee contribution. <br />12.2 January 1 St, 2001 to July 1 St, 2001. A full-time employee participating within <br />either an individual or a family plan shall contribute an amount equal to 12% of the plan premium <br />or the expected cost of such medical plan and the Employer shall contribute 88% of such cost. <br />From July 1't, 2001 through the duration of the agreement, the employee shall pay 13% and the <br />Employer shall pay 87% of such cost. <br />All employee contributions, as required by this Article, sha.ll be withheld via payrnll <br />deduction not later than the first pay period each month for all the employee percentage <br />contributions stated above. <br />12.3 In conjunction with participating within a medical plan, the ernployee may <br />subscribe to and participate in the following optional health benefit coverage(s). <br />l. Dental <br />2. Vision - Optional Plan #1 (Vision plan which was in effect August 1, 1994) <br />Optional Plan #2 ( Vision plan similar to that offered throughout Union <br />Eye Care). <br />3.
