If appdicable, a current dicense or certificate issued by any trade council, <br />testing institution, accredited board or other trade group, together with <br />excrmination results; and <br />5. A copy of any licenses or registration certificates issued by any other state or <br />municipality, including any eaarmination results. <br />(e) Evidence o_f Compliance. All applications for local license and registration <br />certificate shall include the following documentation, to be supplied by the applicant: <br />. , , <br />, <br />, <br />$58;898: 98; <br />2. A certificate of insurance for generad liability for bodily injury and property <br />damage in the amount of $250, 000. 00; <br />3. A certificate of insurance for construction claims and urrworkmanlike <br />performance in the amount of $250, 000. 00; <br />4. A certificate of coverage from the Bureau of Workers' Compensation; and <br />5. Federal taz or payroll identiftcation rtumber or Regional Income Tax Account <br />number or other evidence of registration andlor certification of compliance <br />with RITA withholding. <br />(fi Issuance of Certificate. The Building Commissioner, upon receipt of all required <br />fees, documentation and evidence of compliance necessary to satisfy the provisions <br />herein, shall issue or derry a certificate of registration for a State licensed contractor <br />pursuant to subsections (c) and (e); shall issue or deny a local license certificate <br />pursuant to subsection (d) and (e); or shall issue or derry both forms of certification to <br />qualified applicants. An application shall not be deemed to have been approved untid the <br />Building Commissioner has examined such application, determined therefrom that the <br />appliccmt has satisfted the requirements of Section 1317.04, and has issued a certificate <br />for docal license or a certificate for registration of State licensee or both. <br />(g) A eads. The applicant for a local license or regisiration certificate may appeal <br />from the denial by the Building Commissioner to the Board of Buidding Code Appeals. <br />The Baard shall hear and decide the appeal at its next regular meeting. At the discretion <br />of the Board of Building Code Appeals, after a review of records and an oral <br />examination, a temporary local license certificate may be issued but not to exceed two <br />years in duration. After the two year probationary period, the applicant may reapply to <br />the Buidding Commissioner and, upon appeal, the Board shall either deny or affirm the <br />application. An applicant for a local license certificate or a probationary local licensee <br />may obtairt a State license or certification and also appdy for registration thereof under <br />subparagraph (c), which registration shall be independent of the local license <br />requirement with respect to the work or trade so licensed by the State.
