(b) Sworn Ap,plication. All applications for local

<br />reasonable standards determined by the Building Commissioner, renewal forms may be

<br />(c) Registration Submittads. All applicants for a certificate of registration must
<br />provide evidence of compliance with Revised Coale Section 1317.05, which shall be
<br />deemed satisfactory upon the presentation of documentation demonstrating a current
<br />license or certi, fication issued by the State of Ohio in a form under seal or a copy thereo, f.
<br />No , further or additional testing, review or evidence of skill shall be required of a
<br />contractor demonstrating such compliance with said State law, provided however that the
<br />evidence of compliance is also submitted as required in subparagraph (e) herein.
<br />(d) License Submittals. All appdicants for a local license must provide satisfactory
<br />evidence of skill in the work or trarde for which a license is sought, together with other
<br />documentation required herein. Satisfactory evidence of skild must be demonstrated to
<br />the Building Commissioner, by submission of any or all of the following:
<br />1. An Affidavit from the Applicant which describes the manner in which he has
<br />been actively engaged in the licensed area.for a period of five or more years
<br />and which otherwise describes his expertise, including formal education,
<br />testing or specialized training and certification;
<br />2. A complete list of the names, addresses and phone numbers of cdients or other
<br />contractors for whom they have performed work in this or any trade and a
<br />description of the projects involved;
<br />3. If applicable, a current license or certi, fication by the State of Ohio in any
<br />field or trade;
