''.. .5?:.,1M .: ..: •• . . ..aa?nn' ..6'?t , ..,?bNw:iK?rbefi?ii.'.. 5x?.?-'+•iwf.?'.i?.Cr .:a+.a <br /> S.?£.?.: v +4s w x?3a'9&N3 W.? «... <br />10,06 If an employee is terminated (voluntarily or involuntarily) priar to taking his vacation and <br />has completed one (1) year of employment with the Employer, he shall receive the prorated <br />portion of any accrued but unused vacation at the tinle of separation. ln case of death of an <br />employee, the unused vacation leave shall be paid to the estate. <br />10.07 (A) An employee may have the option of using a personal day as a day of extended <br />vacation, with prior approval of the Chief of Police or his designee. <br />(B) The Employer may have the option of requiring the employee to take one week of <br />his vacation in increments of not less than five (5) consecutive days. The reinaining vacation <br />iime can be taken as needed by the enlployee, for example, one (1) ar two (?) days at a time. An <br />employee will give the supervisor at least forty-eight (48) hours advance notice in writing. In an <br />emergency situation, verbal communication with the supervisor is appropriate and nzay j ustiify <br />waiving advance notification. <br />ARTICLE 11 PROBATIONARY PERIOD <br />11.01 The probationary period for all new employees shall ccintinue for twelve (12) months, <br />beginning on the date of hire. During this probationary period, the employee shall have no rights <br />to appeal any form of disciplinary action, including termination through the Grievance ProcedLire <br />or to anv Civil Service Commission. <br />ARTICLE 12 HEALTH iNSURANCE <br />12.01 The Employer shall provide, pursuant to Appendia A, either individual or family medical <br />insurance coverage at the employee's option for each full-time employee. The insurailce <br />coverage in effect in 2006 shall continue through 2007. The Employer shall have the option of <br />seeking alternate health insurance providers or plans. <br />12.02 Effective January 1, 2008, employees shall pay ten (10%) pei-cent and the Employer <br />shall pay ninety (90%) percent of the insul-ance plan costs. <br />All employee contributions, as required by this Article, shall be withlleld via payroll <br />deduction not later than the first pay period each month for all the employee percentage <br />contributions stated above. <br />12.03 Employees shall participate in the AFSCME Benefit Plan at the present level of benefits. <br />1. Vision - Optional Plan #1 (Vision plan which was in effect August 1, 1994) <br />Optional Plan 92 (Vision plan similar to that offered tlu•oughout Union Eye <br />Care). <br />12.04 Expected cost or premium is defined as the cost of all medical, liospital, prescription <br />drug, and related fees including, but ilot liinited to administrative fees. <br />12.05 The Enlployer reserves the right to establish all plans, enrollment periods and regLilations.
