ARTICLE 12 <br />HEALTH INSURANCE <br />12.1 The employer sha11 provide either individual or family medical insurance coverage <br />at the employee's option for each full-time employee. The employer shall have the option of <br />seeking altemate health insurance providers or plans and will attempt to offer at least two <br />different plans. Basic haspitalization will be consistent with and similar to those plans offered on <br />July 1, 1994. Basic hospitalization will be that offered to ather employees of the City of North <br />Olmsted. The ernployer has the right to negotiate co-payment and prescription drug rates with <br />providers to lower the cost to the employer as well as the employee contribution. <br />12.2 3anuary 1 St, 2001 to July 1s`, 2001. A full-time employee participating within <br />either an individual or a family plan shall contribute an amount equa.l to 12% of the plan premium <br />or the expected cost af such medical plan and the Employer sha11 contribute 88% of such cost. <br />From July 1", 2001 through the duration of the agreement, the empioyee sha11 pay 13 % and the <br />Employer shall pay 87% of such cost. <br />A11 employee contributions, as required by this Article, shall be withheid via payroll <br />deduction not later than the first pay period each month for all- the employee percenta,ge <br />contributions stated above. <br />12.3 In conjunction with participating within a medical glan, the empioyee may <br />subscribe to and participate in the following optional health benefit coverage(s). <br />l. Dental <br />2. Vision - Optional Plan #1 (Vision plan which was in effect August 1, 1994) <br />Optional Plan #2 ( Vision plan similar to that offered throughout Union <br />Eye Care). <br />3.
